## What is a client intake form?

The questionnaire a salon, spa or studio sends before a first appointment: contact details, what the client wants done, and the history that decides whether it can be done safely — chemical history for a colourist, medical history for an esthetician or massage therapist.

A consultation form goes further and shapes the service itself: current versus desired colour, brow technique, placement and size, pressure level, areas to focus on. An appointment request is the shortest — service, preferred date, technician, new or returning.

## Getting better responses

Send it at booking, not on the morning

Clients have to work out when their last colour was and read ingredients off the bottles in their bathroom. An hour's notice gets guesses.

Ask when, not whether

A yes to 'have you had a chemical service' tells you very little. The date is what your judgement rests on, for colour history and skin treatments alike.

Re-ask rather than reuse

A two-year-old intake knows nothing about a new medication, a pregnancy or a recent procedure. Set a refresh interval and send returning clients the same link again.

What should a salon client intake form ask?

Hair type, thickness and current length, the colour on the hair now and how it got there, and every chemical service with an approximate date — box dye, relaxer, keratin, perm. Then the desired service and the result the client has in mind, ideally with a link to a reference photo. Allergies and sensitivities should be a required field rather than an optional one, and a short 'how did you hear about us' at the end is worth having for marketing.

What is the difference between an intake form and a consultation form?

An intake form is about the person: contact details, history, medical and allergy disclosures, collected once and refreshed periodically. A consultation form is about the service they are booking now — current versus desired colour, brow style, tattoo placement and size, pressure level, areas to avoid. Studios that offer several services often keep one intake and a separate consultation per service type.

Can clients book a time or pay a deposit on the form?
