# Business form templates

Business forms ask about the organisation, not just the person — company name, size, industry and role — so that a submission arrives with enough context to act on. These templates cover the three jobs that recur across a business: capturing and qualifying demand, researching a market, and taking in structured records from clients and suppliers.

They fall into three groups. Demand capture — lead generation, demo requests, the product recommendation quiz — sits at the front of the sales process and decides which enquiry gets a call and which gets a pricing page. Research — market, persona and competitor surveys — sits behind product and positioning decisions. Intake — vendor registration, reference requests, the branding questionnaire — creates one uniform record before a working relationship starts.

What they share is a qualifying layer. A contact form takes a name, an email and a message; a business form adds the questions that make the answer useful to whoever reads it next — company size, industry, the problem they are trying to solve, how they found you. That is the difference between a list of addresses and a pipeline you can segment.

## What is a business form?

A business form runs a commercial process rather than a personal one: qualifying a prospect, registering a supplier, briefing an agency, researching a market. Its defining feature is the organisational context it collects alongside the individual's details. Job title, company name, company size and industry recur across these templates because they are what let you route a submission — to a rep, a category manager, a research segment — rather than simply file it.

The answers are also read by someone who has to decide something, so ask for what that reader needs. A rep wants a use case and a timeline before a demo call. Procurement and accounts payable want a tax ID, a business type and a service category. A designer working from a branding questionnaire needs to know which brands the client dislikes as well as which they admire.

## Why use business form templates

Qualification before the call

Company size, industry, use case and timeline collected at submission mean a rep opens the record knowing whether this is a self-serve trial or a long enterprise cycle. Both the demo request and lead generation templates ask for the use case in the prospect's own words.

Attribution you can act on

Several of these templates put "how did you hear about us" next to company size and industry. That pairing tells you which channels bring the deal shapes you want, not only which bring the most enquiries.

Supplier records in one shape

Vendor registration collects tax ID, business type, years in business and service categories as one record. Otherwise supplier details live in email threads, and nobody can search by category or route a registration to the right buyer.

First-party research, not a bought report

The market, persona and competitor surveys ask your own customers what they use, what they pay and what would make them switch. You choose the competitor names on the checklist, so answers map to a decision you are about to take.

Briefs collected in advance

A branding questionnaire or reference request sent ahead of the kickoff call turns that call into a discussion of the answers rather than a transcription of basics.

## How to create your own

1. 1

Decide which decision the form feeds — a sales call, a supplier approval, a research segment — and list the two or three facts that decision cannot be made without. Those are your required fields.

2. 2

In formformform's document-style editor, type the form as you would write a document: name, work email and company first. Label it work email rather than email, so personal addresses stay out of your pipeline.

3. 3

Add the qualifying block — company size, industry, job title — plus one open question about the problem they are trying to solve. Keep that one free text; dropdowns about challenges produce clichés, open text produces sentences you can quote.

4. 4

Let conditional logic carry the length: branch enterprise-sized companies to timeline and budget, reveal tax ID and service-category fields only to suppliers, show a second and third reference block only when the applicant has more to give.

5. 5

Close with an attribution question, then route the response: a real-time webhook or Zapier action into your CRM, and an email to whoever owns first response. Embed it on the landing page, or share a link for supplier onboarding.

## Getting better responses

Require the work email

A prospect who will not give a work address is rarely a buyer, and consumer addresses are what clog a B2B pipeline. Labelling the field "work email" signals that this is a business enquiry, and it makes company matching easier later.

Cap required fields at five to seven

Across the lead generation and demo request templates the qualifying set is name, work email, company, company size and use case. Make anything beyond that optional rather than deleting it: someone with a real project will fill it in anyway.

Make research contact fields optional, and say so

Competitor and persona surveys only work when people are candid about what your product does badly. Put name and email at the end, optional, and state on the form that a response can be anonymous.

Separate registration from approval

A vendor form collects a profile; it does not approve a supplier. Say so on the form, then verify tax details and references as a distinct step — otherwise someone registers on Monday and invoices you on Tuesday.

Ask what they dislike

Branding questionnaires and competitor surveys get their sharpest answers from negative questions: which brands the client cannot stand, what nearly made a customer switch away. Positive-only questions produce answers that would fit any company.

## Frequently asked questions

What is the difference between a contact form and a business form?

A contact form captures a name, an email and a message so someone can reply. A business form adds the qualifying layer that decides what happens next: company size, industry, job title and what the person is trying to achieve. That context is what lets you route a submission to the right rep, category manager or research segment instead of triaging every enquiry by hand.

What should a B2B lead generation form ask?

Name, work email and company are the baseline. Add company size and industry to segment the lead, an interest question — demo, pricing, free trial, custom quote — to route it, and one free-text question about the problem they are trying to solve. Put an attribution question at the end, where it costs you nothing in conversion.

How many questions should a business form have?

For demand capture, five to seven required fields is the working range: enough to qualify, not enough to deter someone mid-evaluation. Research surveys and vendor registrations can run longer because the respondent has already committed the time, though a persona or market survey should still be answerable in under ten minutes. Use conditional logic so long branches appear only for the people they apply to.

How many responses do I need before market research is useful?

It depends what you will do with it. For directional decisions — which pain point to lead with, which alternative you are really competing against — a handful of responses from exactly the right role, industry and company size beats a large number from a general audience. If you intend to publish percentages, you need a properly recruited sample; a survey of your own list is not a representative panel.

Can I send business form submissions to my CRM?

Yes. Every submission can fire a real-time webhook, and there is a REST API plus 60+ app integrations through the published Zapier app, covering CRMs, spreadsheets and chat tools. Traffic only runs outwards: a submission triggers an action, such as creating a lead or posting a notification. formformform does not read records back out of your CRM.

Do these business templates cost anything?

No — early access is free, with nothing held back behind a plan and no ceiling on forms or responses, and anyone who joins now keeps the early-adopter rate once paid plans arrive. Start from any template here, rewrite the qualifying questions to match your own criteria, and publish.
