## What is a community form?

A community form asks a supporter what they can give — hours, money, expertise, a signature, a name to put forward — and adds enough structure to act on it. ## Why use community form templates

Matching, not just collecting

A volunteer placed in the wrong role costs more time than no volunteer. Interests, hours per week, languages and transport turn a list of names into a placement decision.

Records a funder or a court will ask for

Hours logged per shift with the date, programme name and supervisor become an in-kind match on a grant report, a student's service total or a court verification, not a figure reconstructed at year end.

A front door for people outside the circle

Petitions, event proposals and board nominations reach people who do not know who to ring: the resident with an idea for a mural walk, the company that would sponsor if asked.

Reply while the impulse lasts

Volunteering and giving are decided in a moment. Someone who submits an enquiry on Sunday and hears nothing by Wednesday has found another outlet, so notify the coordinator directly.

Keep screening data off a public form

A background check authorisation asks for identifiers no public page should collect. Send it individually to accepted volunteers, turn on end-to-end encryption, and confirm the consent wording your jurisdiction expects.

Ask for examples, not sentiment

"She goes above and beyond" cannot be weighed against another nomination; "she trained twelve new volunteers this quarter" can. The same holds for the community-benefit answer on an event proposal.

Which questions belong on a volunteer registration form?

Contact details, days and hours available, the interest areas you can actually place people into, relevant skills and languages, and any prior volunteering. Add an emergency contact for anything in person, and a transport question if the role happens off-site. Leave medical detail, identifiers and background check consent to a later stage — a first registration that reads like a screening form loses people who would have been good volunteers.

How do we collect background check consent safely?

Send the authorisation form individually to volunteers you have already accepted rather than publishing it, and ask only for the identifying details your screening provider actually requires. End-to-end encryption and EU hosting are available, and submissions are encrypted at rest and in transit. Consent wording and disclosure rules differ by jurisdiction, so confirm what applies to your organisation before you run checks.

How do we track volunteer hours for a grant report?

Use a short log filed after each shift: date of service, hours, the programme or project name, the supervising staff member, and a line on what was actually done. Make the programme a dropdown rather than free text, so spelling variations do not fragment the totals. Responses can then be filtered, searched and exported for the reporting period.

Can we collect donations or membership dues through a form?

Yes. Add a Stripe-powered payment field when you want to collect a gift or renewal immediately alongside the amount, fund designation and any dedication. A pledge form is different: it records an intention so someone can follow up personally, so it can remain payment-free.
