In Contact Forms

- [Quote request form templates · 10](https://www.formformform.com/form-templates/contact-forms/quote-request)

Most sites treat the contact page as one destination: a single form feeding a single undifferentiated inbox. These templates assume the opposite — a journalist on deadline, a supplier pitching a catalogue, a customer with a billing grievance and a prospect sizing up a project all want different things and deserve different questions.

The collection splits along intent. The open door asks a name, a way to reach you and an open message, nothing else. Qualified inbound adds context: quote requests with a budget range and timeline, callbacks with a preferred time slot, business enquiries with industry and company size. And some enquiries arrive with their own protocol — press requests with a deadline, vendor and partnership pitches with a website to check, complaints with a date of incident and a desired resolution.

## What is a contact form?

A contact form is a short block of fields on your website that lets a visitor send you a message without leaving the page or opening their mail client. It captures who they are, how to reach them and what they want, and stores the submission so nothing depends on one person remembering to forward an email.

The interesting part is the third of those. "What they want" can be a free-text box, or a structured question that changes what happens next — a reason-for-call dropdown that decides who rings back, a request type that separates an interview from a press-kit request, a service type that decides which quote you prepare. That choice is what separates a contact form from an inbox with extra steps.

## Why use contact form templates

A form gets filled in; an email address gets copied

An address on a contact page asks the visitor to switch apps, invent a subject line and guess what you need to know. A form does that thinking for them.

Context arrives attached to the message

"Interested in working with you" takes three emails to qualify. The same enquiry with an industry, a timeline and a budget range attached is already triaged, so your first reply can be an answer rather than another question.

One form can behave like several

Conditional logic branches on earlier answers. Open with "what brings you here?", then reveal only what that intent needs — an outlet and deadline for press, an event date and audience size for a speaking request, a date of incident for a complaint.

Sensitive messages need somewhere proper to land

Complaints, workplace grievances and candid staff suggestions arrive through contact forms whether you planned for them or not. Optional name and email raise the volume of honest feedback, and a form carrying personal detail can be end-to-end encrypted and EU-hosted. Confirm what your own sector requires.

Each submission can set something in motion

Real-time webhooks fire on submit and 60+ apps connect through the published Zapier app, so a vendor pitch opens a task for procurement while a quote request creates a CRM record. The submission triggers the action, which is what enquiry routing needs.

## How to create your own

1. 1

Write the form the way you would write a note: a one-line intro in your own voice, then name, a way to reach them, and an open message box.

2. 2

Add the one structured question that decides everything downstream — reason for call, enquiry type, service type or nature of complaint — with options specific to the work you take on.

3. 3

Branch from it with logic: a deadline for press requests, a budget range for quotes, a call slot for callbacks, a date of incident for complaints, each hidden from everyone else.

4. 4

State your response time in the form itself, and add a short text field wherever a useful reply depends on seeing a brief, a portfolio or a catalogue — a link to a live site or a shared folder arrives with the message.

5. 5

Embed it on your contact page in two lines of HTML, or run it as a popup or slide-in where the question comes up, then webhook the submissions to whoever answers them.

## Getting better responses

Ask for a phone number only when you intend to ring

Callback and vendor enquiries genuinely need one — a procurement conversation is a phone conversation, and asking later costs a round trip. On a general contact form it is friction with no payoff.

Include the budget question, framed as a range

Budget is the field clients most dislike and service providers most need. A range with an "unsure" option reads as scoping rather than commitment, and paired with a timeline it tells you which enquiries to answer first.

Give complaints their own route

A grievance sent through the same box as a newsletter question feels dismissive, and a complaint form that is hard to find becomes a public post instead. Link one clearly from your contact page, ask for the date of incident and the resolution sought, and acknowledge receipt.

Make the sender's website a required field on any pitch

Partnership, vendor and speaking enquiries all arrive with self-reported credibility. Ten seconds on the sender's site settles more than a dropdown ever will — audience, scale, past clients, whether the event exists.

Let feedback stay anonymous

On suggestion and feedback forms, keep name and email optional and add one consent question for follow-up. People say the useful thing when there is no social cost to saying it, and anyone wanting a reply will volunteer an address anyway.

## Frequently asked questions

What should a contact form include?

A name, a contact method and an open message field cover most general enquiries. Add a subject or enquiry-type question as soon as more than one person reads the submissions, because that is what makes routing possible. Everything past that should earn its place — each extra field is another point at which someone abandons the form.

Should I use one contact form or a separate form for each type of enquiry?

Both work, and the deciding factor is whether the enquiries need genuinely different questions. A press request needs an outlet and a deadline; a quote request needs a budget and a timeline; neither belongs on the other's form. Either publish separate forms on separate pages, or run one form that branches on an opening "what brings you here?" question.

Where should a contact form go on a website?

A dedicated contact page is the baseline, since that is where visitors look for it. Beyond that, put the form where the question arises — a quote request on the services page, a callback request as a slide-in on pricing, a press form linked from the newsroom. The same form can be embedded on several pages at once, or shared as a link or QR code where a web page is not practical.

How do you cut down on junk contact form submissions?

Specific questions do most of the work. Asking for a company name, a work email, a website or a project timeline takes more effort than a scattergun pitch is worth, and it gives you something to check. Saying plainly in the intro line what you do and do not want to hear about filters a surprising amount before anyone starts typing.

Can contact form submissions be kept confidential?

Yes. A form carrying complaints, workplace grievances or embargoed press detail can be end-to-end encrypted and hosted in the EU, and submissions are encrypted at rest and in transit. formformform is GDPR compliant. Confirm what your own sector or jurisdiction requires of you before you start collecting sensitive detail.

Are these contact form templates free to use?

Yes. formformform is free during early access, with every feature unlocked, unlimited forms and unlimited responses. Early users keep early-adopter pricing when paid plans launch. Open any template in this collection, rewrite the fields to match how you work, and publish it.
