# Return and refund request form templates

A return or refund request form has to establish three things before anyone can act on it: which order or account it concerns, why the customer wants out, and what they want back. Every form on this page does the same job in a different currency. Someone bought something, it did not work out, and they need a structured way to say so — which order or account, what went wrong, and what they want to happen next. That job does not sit in one part of a form library: the general refund, warranty and cancellation templates live in formformform's customer support set, while the product return and size exchange templates sit in retail, because a shop writes the same request around packaging, tags and sizes rather than around a ticket. Grouping them here lets you compare the five side by side instead of picking whichever one your category page happened to show you.

The spine is shared and boring: an identifier, a reason, and a resolution. The identifier is an Order Number on three of them, a Serial Number or Model Number and Place of Purchase on the warranty claim, and an Account Number or Username on the cancellation — optional there, since the required email address on the account already identifies it. The reason is a dropdown on three, and the option lists are where they genuinely diverge, because "Doesn't fit" and "Damaged in shipping" appear only on the return form's Reason for Return, "Item never arrived" and "Duplicate order" only on the refund form's Reason for Refund, and "Too expensive" and "Found a better alternative" only on the cancellation's Reason for Cancellation, the one list that is about the service rather than an item. The other two ask for the reason in prose: the size exchange takes it as a required Reason for Exchange text box, sitting below the two size fields that have already answered it, and the warranty claim as a required Description of Defect or Issue. The resolution question is the real chooser: Refund to original payment method, Exchange for same item or Store credit on the return; Original payment method, Store credit or Exchange for another item on the refund; Repair, Replacement or Refund on the warranty claim; a shipping sequence with a store-credit fallback on the exchange; and on the cancellation, an effective date instead of a resolution at all.

The constraint that bites hardest here is attachments. Returns run on evidence — a photo of the cracked screen, a picture of the wrong item in the box, a receipt — and no template on this page arrives with a place to put one, because formformform's template generator cannot seed an upload field. The warranty claim makes this visible: it asks "Do you have proof of purchase available?" as a required yes-or-no radio and then has nowhere for the yes to go. ## How to build a return and refund request form

1.  Write down the resolutions you actually honour — money back, exchange, store credit, repair, or nothing but a cancellation date — before you write a single field, because that list is the question the rest of the form exists to support.
    
2.  Ask for the identifier the customer can genuinely find: an order number and purchase date for a shop, a serial or model number and place of purchase for a warranty, an account username for a subscription. Asking for all three guarantees an abandoned form.
    
3.  Build the reason list from your own returns data rather than a generic one — "Doesn't fit" earns its place on apparel, "Item never arrived" belongs on a form that absorbs shipping failures, and "Found a better price" only if you actually price-match.
    
4.  Add the one eligibility question your policy turns on: original packaging, tags still attached, or whether proof of purchase exists. Those answers decide approve-or-deny before anyone opens the request, and they are cheap to ask and expensive to chase later.
    
5.  Add a File Upload field in the editor after you clone the template so photos of the damage and a copy of the receipt arrive with the request, and add branching there too if you want the exchange questions hidden from someone who asked for a refund.

## Frequently asked questions

Can customers attach a photo of the damaged item or their receipt?

Not from these templates as generated, but yes once you clone one. formformform's editor has an Upload section — up to 2 MB per file, encrypted along with the rest of the response on an encrypted form — it is only the template generator that cannot seed the field, which is why the warranty claim asks whether you have proof of purchase without offering anywhere to attach it. Drag an Upload field in beside the description question and the evidence arrives with the request. If you would rather not receive the files at all, a Short Text field asking for a link to a shared album does the same job.

Can the form refund the money, or charge a restocking fee?

You can add a Stripe-powered payment field when the form needs to collect a restocking fee or another charge. Refunds still go back through the processor that took the original payment; these templates collect the request and the decision inputs, while the refund itself is issued from that original transaction.

Should returns and refunds be one form or two?

One form works when the goods are always coming back and the only open question is what the customer gets in exchange — that is exactly what the Product Return Request Form does, with a Preferred Resolution radio offering refund, same-item exchange or store credit. Split them when refunds happen without a return: the Refund Request Form exists because "Item never arrived" and "Duplicate order" are refund cases with nothing to ship, and its "Return not applicable" answer would be nonsense on a returns form.

Is an account cancellation the same thing as a refund request?

No, and it is worth keeping them apart. A cancellation is an exit from an ongoing agreement, so the Account Cancellation Request asks for an account identifier and a Requested Effective Cancellation Date and never mentions a product, an order or a purchase date. It also asks two things a returns form has no reason to — whether to export the customer's data before the account closes, and whether you may contact them about a retention offer. If a cancellation also owes someone money back, that is a separate refund request against a specific charge.

Can I hide the exchange questions from someone who only wants a refund?

Yes, but not out of the box. Every field in these templates is generated flat and always visible, even though formformform's editor supports branching a form on an earlier answer. After you clone, point the Preferred Resolution question at whatever only applies to one branch — the packaging question on a return form, the size fields if you have folded exchanges into it — so nobody answers a question their chosen resolution does not need. It is a few minutes in the editor, not a missing capability.
