Every template below asks for a name and a way to reach the registrant, then spends the rest of the form on what makes that particular event different: which days and sessions for a conference, which pass type and date of birth for a festival, no dietary or accessibility question at all for a webinar because there's nowhere to physically be. Match the template to the shape of your event rather than the other way round, and most of your customising time goes into editing dropdown options rather than building fields from nothing.

Two questions show up on most of these forms under a different name depending on which template you're looking at: dietary needs and accessibility. One template asks for "Accessibility Requirements" as free text, another for "Accessibility & Accommodations" against a checklist of specific options, another for "Accessibility Needs" as an open paragraph; dietary restrictions carry a plain label on some forms and a "(if catered)" or "(for catered meals)" qualifier on others. That isn't an oversight — a checklist of named accommodations suits a workshop with a handful of seats better than a festival selling thousands of tickets — but it does mean any field count you see for these two questions is counting near-duplicate labels separately rather than one merged concept.

None of the templates below asks a plain "will you be attending?" — they're built for people who haven't already been personally invited, which is what puts them in this collection rather than with the RSVP templates, a Party RSVP Form or a Wedding RSVP Form, built around exactly that question and an already-known guest list. And none of them ships with conditional logic already wired in: formformform's template generator writes the fields but not the branching, so showing the conference's dietary question only to in-person attendees, or revealing the gala's Guest Names field only when someone books a couple or a full table, is something you set up in the editor after cloning, not something the template arrives with.

## How to build an event registration form

1.  Pick the template above whose central choice already matches how people register for your event — ticket tier, attendance type, workshop, pass type or table size — rather than starting from whichever template happens to be nearest the top.
    
2.  Clone it and rewrite the options on the field that carries that choice — Ticket Type, Workshop, Pass Type or Reservation Type — so they match your actual tickets, workshops or passes. The Webinar and Trade Show forms have no ticket field at all: there the first dropdown is Company Size or Industry, and those are the option lists to edit.
    
3.  In the editor, add conditional logic for anything that doesn't apply to everyone — skip the dietary field for virtual attendees, or reveal a guest-name field only when someone books a couple or table reservation. None of these templates ships with that branching built in.
    
4.  Keep protective fields — emergency contact, allergies, accessibility — required rather than optional, and leave reflective ones like "what do you hope to get out of this" optional. A required field costs a registrant nothing to answer and costs you a lot to be missing.
    
5.  Set a confirmation message or redirect that tells people what happens next, and connect the form to wherever the rest of the event runs — a shared sheet, a Slack channel, your ticketing tool — through a webhook or an integration, so nobody has to re-type the list.
    
6.  Share the link or embed the form on the event page, and if you're worried about selling out, watch the response count as entries come in and swap in a short waitlist form once you're full.

## Frequently asked questions

What's the difference between these and an RSVP form?

These are for people you haven't personally invited yet, so most of them carry a ticket, pass or registration type, and the Festival form adds a required age check and a waiver on top of that — none of which a known guest list needs. If you already know who's coming and just need a yes/no plus a headcount, use the Party RSVP Form or Wedding RSVP Form instead — both are built around a single attendance question rather than a ticket or pass.

Can I hide fields that don't apply to some registrants, like the dietary question for virtual attendees or guest names for single-seat bookings?

Yes, but not automatically. None of the templates here ships with that branching already set up — you add it in the editor after cloning the form, using a radio or dropdown answer such as attendance type or reservation type as the condition. It's a couple of minutes of setup, not something the template arrives with.

Do any of these collect payment for tickets or table reservations?

Yes. Add a Stripe-powered payment field to collect a registration fee in the same form that records a ticket tier, pass type or table size. For events with reserved inventory or prices that need approval, you can still use your ticketing or invoicing tool after the submission.

My event doesn't match any of these exactly — which one should I start from?

Start from the Event Registration Form if your event is a single day with a straightforward ticket tier and nothing else distinctive; it's the least specialised template on this page and the easiest to adapt. Otherwise pick whichever template shares the most with your event — a two-day workshop series is closer to Conference than Workshop, a ticketed outdoor fair without camping is closer to Festival than Event Registration — and remove whatever doesn't apply.

Which one handles a multi-day event — Conference or Festival?

It depends what people are choosing between. Conference Registration Form is built around sessions and tracks: people pick which days and which talks, with a badge name and continuing-education credit for a professional audience. Festival Registration Form is built around passes and add-ons: people buy a single day, a weekend or a camping pass, with a required emergency contact and an age check. If your event sells tracks, use Conference; if it sells passes, use Festival.
