## What is a government form?

What marks a government form out is the clock. Every exchange between a public body and the public carries a deadline on one side or the other: a comment period closing on a published date, a records request starting a response clock, an event permit needing thirty to ninety days to reach police and fire. The submission is often a public document itself, so it should ask only what the decision needs.

## Getting better responses

Put the deadline and the emergency route on the form

Residents cannot see your calendar. Write the comment close date, or the minimum notice for an event permit, into the form. On anything describing a hazard, print an emergency number at the top.

Write the lists for your jurisdiction

Every option missing from a dropdown becomes an 'Other' that costs a clarification call. List the permit types you actually issue, the boards with seats free, this week's agenda items.

Assume the submission is disclosable

Permit files and public comment are often public records themselves, so think twice about dates of birth or ID numbers. A sensitive form can add end-to-end encryption and stay on EU infrastructure; retention duties stay yours.

## Frequently asked questions

Are public comments submitted online part of the official record?

Agencies generally treat written comment received before the deadline the same way regardless of how it arrived, and many read or summarise it into the record at the hearing. A form helps by making attribution possible: a required name, a contact detail, the agenda item, and a timestamp showing the comment arrived before the period closed. Your clerk's rules and your state's open meetings law set the actual requirements, so confirm those before changing your process.

What has to be on a public records request form?

A description of the records specific enough to search against, a date range, the requester's contact details, a format preference for digital or paper, whether the use is commercial, and a fee waiver request with a justification field. Specific descriptions are fulfilled far faster than 'all emails from the mayor', so use the form's own intro text to explain what a workable request looks like. Response deadlines and fee rules differ by jurisdiction — check the law you operate under.

Can residents attach photos, site plans or drawings?
