This page covers one task: what a patient fills in before a first appointment. It leaves out the other jobs healthcare-forms bundles together under the same category — prescription refill, records and equipment requests from patients who are already on file, generic appointment booking, and venue health-screening questionnaires. Those ask for different things from different people, and folding them into one comparison would hide more than it shows.

## How to build a patient intake form

1.  Start with identifiers: full name, date of birth and at least one contact method. If you'll only ever call or only ever email, require just that field and leave the other optional — forcing both is one of the fastest ways to lose someone at the top of a short form.
    
2.  Decide new-or-returning or adult-or-child early. Once you've cloned a template into your own account, add conditional logic in the editor so a guardian-name field or a first-visit history block only appears when it's actually needed — none of the templates below ship with that branching built in, so it's a step you add yourself.
    
3.  Add only the clinical section your discipline actually uses — medication and allergy questions for general practice, a pain scale for chiropractic, a symptom checklist for behavioural health — and for every field, name the decision someone on your team will make with the answer. If there isn't one, cut the field.
    
4.  Keep insurance, payer and emergency-contact fields optional unless your policy genuinely requires them from every patient. Most of the templates below already default that way, and a card or contact detail nobody has to hand is the fastest way to stall a first-time patient at question four.
    
5.  Add a consent or privacy-notice acknowledgement next to whichever section is most sensitive, rather than leaving it to a footer link — it's the field most intake templates here skip entirely, and a single required checkbox is enough to add it.
    
6.  If you're collecting anything a patient would consider genuinely sensitive, turn on end-to-end encryption for your own copy before you share the link, and confirm your own legal and regulatory obligations separately — no template, encrypted or not, makes a practice compliant on its own.

Which patient intake template should I start from?

Start with the general patient intake form if you want one form to cover demographics, an emergency contact and insurance in a single pass — it's built for a front desk handling a first-time visit from scratch. If scheduling and payer details already live in a separate system, the medical history form goes deeper into medication, family and lifestyle history without repeating them. The dental, vision, chiropractic and speech therapy templates are each built around the one or two questions specific to that discipline — a pain scale, a glasses-or-contacts question, a guardian field — rather than a generic history section, so pick by specialty first and adjust the rest afterwards.

Why do the templates that ask for an insurance provider or policy number leave those fields optional?

Because not every patient filling in an intake form has an active policy to hand at that moment, and a required field nobody can answer is exactly what makes people abandon a form. The templates below that ask for a provider name or policy number leave it optional, and some collect a simpler payment-route question instead — self-pay, sliding scale, insurance, EAP. If your practice needs insurance confirmed before the visit, make the field required in the editor after cloning — just be aware you're choosing to add friction most of the live templates deliberately avoid; the chiropractic intake is the one that does require a yes/no on coverage.

Can I show a guardian field only when the patient is a child?

Do these templates keep sensitive health answers private?

What is the difference between patient intake and patient registration?

Registration opens the record: who the person is, how to reach them, who pays. Intake is the clinical half — why they are coming and what you need to know before you see them. Most practices collect both at once, which is why there is no separate registration template here. The Patient Intake Form is the member that carries both halves: Home Address, Biological Sex, a required emergency contact with a stated relationship, and Insurance Provider, Policy / Member Number and Group Number sit alongside Reason for Visit and the medication, allergy and conditions questions. The Medical History Form is the opposite — sixteen questions with no address, no insurance and no emergency contact, because it is built to be sent to someone who is already on your books. And the Privacy-First Patient Intake Form deliberately has neither the insurance block nor a required date of birth, on the argument that a first visit rarely needs them. If your front desk already holds the registration details in a practice-management system, start from one of those two rather than re-collecting what you have.
