formformform's human resources templates cover the whole employment lifecycle: a requisition to open a role, timesheets and reviews once someone is in it, an exit interview when they leave. This collection pulls out the one step where the person filling in the form does not work for you yet, and has to be compared fairly against everyone else who applied. That is why its members come from across the library rather than from human-resources alone — the questions that actually decide a hire are questions about the work, so the kitchen application sits with the restaurant templates, the driver application with transportation, the counselor application with childcare, and each one carries the vocabulary of its own trade.

What separates these templates is what gets checked before experience does. The general Job Application Form asks for Highest Level of Education, Work Experience, Relevant Skills and a Cover Letter, then leaves the judgment to whoever reads it. Every other member but one front-loads something that can rule a candidate out on its own: Commercial Driver's License Class and a five-year incident list on the driver form, Do you hold a current Food Handler or Food Manager Certification? in the kitchen, Are you current on CPR and AED certification? for instructors, Are you 18 years of age or older? in retail, Work Authorization Status plus two consent questions on the staffing registration. The exception is the online course instructor application, which carries no disqualifying question at all: credentials in prose, then a proposed course title, an outline and an expected length, because there the applicant is pitching work rather than clearing a bar. The second axis is availability, and it is asked in four different shapes across this set — as a checkbox of days or shifts (retail, staffing, coach), as separate weekend and evening radios (kitchen), as one free-text line of days and times (fitness instructor), and as a three-way commitment question — full summer, partial with weeks to specify, or not sure yet (camp counselor). Not one of these templates has a place to attach a CV. formformform's template generator has no file field, so every application here asks for the content of a resume rather than the document: Work Experience and Relevant Skills as long text on the general form, Coaching History on the coach form, Describe Your Retail Experience on the retail one. ## How to build a job application form

1.  Write down the single fact that disqualifies a candidate before you write any questions — a license class, a live certification, a minimum age, work authorization — and make it a required field near the top, so a no is a no before either of you spends an hour on it.
    
2.  Ask for availability in the shape you schedule in: a days checkbox if you build a weekly rota, separate weekend and evening questions if those shifts are staffed differently, one free-text line if hours get negotiated per person, and a single season-long yes/no if you hire in blocks.
    
3.  Replace "attach your CV" with the two or three long-text questions you would have opened the CV to answer — relevant experience, a skills list, and one question that shows how the person thinks about the work, the way the fitness instructor template asks for a teaching philosophy.
    
4.  Collect each consent as its own required radio or checkbox — background check, drug test, driving record — rather than folding them into a paragraph of small print, so what the candidate agreed to is a discrete answer you can point at later.
    
5.  Add a File Upload field in the editor after cloning if you do want documents, and add branching there too: reveal the license questions only when someone picks a driving role, or the certification question only for the roles that require one.
    
6.  Turn on end-to-end encryption before you publish if applications will carry dates of birth, license numbers or consent answers, and webhook new submissions into wherever you track candidates so nothing waits for someone to open the dashboard.

Can applicants attach a CV or resume to these templates?

Not as the templates arrive — formformform's template generator has no file field, so nothing in this collection ships with an upload. The product does have one: clone the template, add a File Upload field from the editor palette, and label it. It takes one file per field at 2 MB each, so use two fields if you want a CV and a cover letter separately, and it is encrypted along with the rest of the response on an encrypted form. If you would rather not host documents at all, do what the online instructor template does and ask for a link in a short text field.

What should a job application form ask that a CV doesn't already answer?

The things a candidate would never think to put in a CV and you cannot hire without: when they can actually work, whether a required certification is current today rather than the year they earned it, whether they are legally eligible to work where the job is, and whether they consent to the checks you run. Those are exactly the fields the specialist templates here add on top of the general one — Days Available to Work, Is your DOT physical current?, Work Authorization Status, background check consent — and they are the fields that let you sort a pile of applications without reading all of them first.

One form for every role, or a separate form per role?

If the disqualifying questions differ, use a form per role: a driver application that skips the license number screens nobody, and a retail applicant should not be asked about a DOT physical. Do these templates come with conditional logic already set up?

No. The template generator writes every field flat and always visible, even though the editor supports branching one field on an earlier answer. So the certification question, the reference block and the license follow-ups all show to everyone until you change it. Clone the template and add the logic yourself — showing the violations field only to applicants who answered no to a clean driving record is a two-minute edit, not a rebuild.

Applications hold dates of birth and license numbers — how should that be handled?

Turn on optional end-to-end encryption for the form before you publish it. It is opt-in per form, so it is a decision you make on the application specifically rather than for your whole account, and it covers uploaded files too if you add a CV field. formformform is GDPR compliant and hosted in the EU. Beyond the product, keep the same discipline the fields imply: ask for a date of birth only where you have a reason like a minimum age, and delete the responses for candidates you did not hire once your own retention window is up.
