# Marketing form templates

Most point inwards. The person filling one in is a colleague or client asking marketing to make something, so the fields ask for a requester name and email rather than a customer's details, and campaign name, target audience and start and end dates recur throughout. They turn "we need something for the launch" into a request that can be sized, costed and queued.

A few point the other way: the agency inquiry and SEO audit forms qualify inbound work before anyone books a discovery call, the influencer partnership application takes the same data from every creator who applies, and the campaign performance report captures spend, conversions and learnings that would otherwise stay in one marketer's head.

## What is a marketing request form?

A marketing request form is where campaign and creative work gets specified before anyone starts making it. It replaces the email or Slack message with a fixed set of questions: what the campaign is called, what it should achieve, who it targets, which channels are in scope, what it can spend, when it goes live and when it needs sign-off.

The specifics change with the deliverable. An email campaign request needs a send date, an audience segment and the exact call-to-action label and destination URL. An advertising request needs the ad format, placement list and creative asset status. A press release request needs a proposed headline, a pre-approved quote and a clear answer on embargo. The shape holds throughout: identify the requester, classify the request, scope it, date it, link to what already exists.

## Why use marketing form templates

Requests arrive scoped instead of vague

An objective dropdown, a budget range, a channel list and a pair of dates are the difference between a brief and a wish. With those four on the form, the receiving team can size the work and challenge an unrealistic deadline the day it lands.

The request type decides where it goes

Nearly every form here opens with a classification: content type, ad format, email type, story category, announcement type. That answer is the routing key. Conditional logic uses it to show only the questions that apply; a webhook or Zapier action uses it to open the submission as a ticket in the right queue.

Inbound enquiries qualify themselves

The agency inquiry and SEO audit forms do work a discovery call would otherwise do. Services needed, budget range, current challenges and named competitors let a proposal be written against something concrete, and prospects who cannot name a goal filter themselves out.

Applicants become comparable

An influencer partnership application gives you platform URLs, follower tier, engagement rate, content niche and past collaborations in the same shape from every creator who applies. A column of engagement rates can be ranked; a folder of direct messages cannot.

Campaigns leave a record you can compare

A campaign performance report fixes the metric set — spend, impressions, clicks, conversions, revenue — alongside the learnings. Because every marketer answers the same fields, responses can be filtered, searched and exported as one dataset instead of a folder of slide decks.

## How to create your own

1. 1

Start from the requester: name, email, campaign name, and a request-type dropdown — blog post, newsletter, paid social, display advertising, press release.

2. 2

Branch on that dropdown with conditional logic: platforms and post formats for social, embargo and quote sources for a press release, placements and landing page URL for advertising.

3. 3

Add the scoping block every marketing request needs — objective, target audience, key message or call to action, and a budget range offered as tiers rather than an empty number box.

4. 4

Ask for two dates, not one: the go-live or send date, and the deadline for sign-off. Add a short text field for the folder holding brand guidelines, existing creative or a draft.

5. 5

Embed the inbound enquiry forms on your site, share internal request forms by link, and point a webhook or Zapier action at your project tool so each submission opens a ticket.

## Getting better responses

Ask for the objective before the deliverable

Requesters arrive knowing what they want made, not what it is for. Put a campaign objective dropdown — awareness, lead generation, retention, launch support — above the format question. It is the field most likely to change what the team produces.

Offer budget as a tier, not a text box

A free-text budget field gets skipped or filled with a placeholder; a dropdown of ranges gets answered. On an inbound agency or SEO enquiry form the tiers also screen for you: anyone who sees them and still submits has priced themselves in.

Ask what already exists before asking what to make

A creative asset status field — ready, partially ready, needs full production — sets the timeline on an advertising or social request more than any other answer, and stops the team producing a hero image the requester already had. Pair it with a short text field for the folder those assets sit in.

Make embargo an explicit choice

On press release and media pitch requests a blank embargo date is ambiguous, and the cost of that ambiguity is an announcement going out early. Use a required radio — "no embargo" or "embargo until" — and reveal the date and time only when the second is chosen.

Close the loop with a fixed metric set

Resist customising the performance report per campaign. Keeping spend, impressions, clicks, conversions and revenue identical every time is what makes the numbers comparable, and collecting them within days of the campaign ending catches the context while it is fresh.

## Frequently asked questions

Which fields does a marketing request form need?

At minimum: requester name and email, the campaign or project name, a request type, the objective, the target audience, a budget range, the go-live date and the sign-off deadline. Add a call-to-action field wherever the deliverable has a destination. For assets, use both: an upload field for a logo or a one-page guideline PDF under 2 MB, and a short text field for the Drive or Dropbox folder holding the full creative library. Either way it arrives with the rest of the brief. Everything else should be conditional on the request type, so a newsletter request never shows out-of-home placement options.

Should I ask for a budget on an inbound marketing enquiry form?

Yes, as a visible set of ranges. Listing tiers signals that budget is part of the conversation and lets you sort follow-ups by revenue potential without a call. If you are worried about drop-off, leave the field optional and keep it on the form anyway — the enquiries that answer it are further along than the ones that skip it.

How far in advance should a campaign or content request be submitted?

It depends less on the channel than on whether the creative already exists. A social post using approved assets can turn around in days; an advertising campaign that needs production, a landing page and a paid budget approval needs weeks. Put a creative asset status field and an approval deadline on the form, and let those two answers set the expectation rather than a blanket rule nobody follows.

What is an embargo date, and does a press release request form need one?

An embargo is the date and time before which journalists who have been briefed early agree not to publish. Any press release or media pitch intake form should ask about it explicitly, because the cost of an ambiguous answer is a confidential announcement going out ahead of schedule. Make it a required choice between "no embargo" and a specific date and time rather than an optional field.

How do I get different request types to the right team member?

Use the classification dropdown at the top of the form as the routing key. Conditional logic branches the questions that follow, and a real-time webhook or one of the 60-plus Zapier integrations fires on submission to create a task, post to a channel or add a row wherever your team works. The flow is one-directional — a submission triggers the action in the other app — so treat your responses view as the record and the destination as the notification.

Is there a charge for using these marketing templates?

No. Early access unlocks the lot — conditional logic, multi-page forms, webhooks, integrations, custom CSS — with no cap on forms or responses. Teams who join now keep the early-adopter rate once paid plans arrive.
