## What is a photography client intake form?

The questionnaire a photographer sends once a session is booked. Half of it is logistics — shoot type, date, location, number of subjects — and half is direction: must-have shots, wardrobe plans, a mood board link, and the reason the photographs are being taken.

The package enquiry before it exists to qualify; the shoot questionnaire after it goes deeper on style and story for one session.

## Getting better responses

Time each form to the shoot

The intake goes out immediately after booking, while the client is still excited. The questionnaire goes out at least a week before, so there is time to plan outfits. A usage request lands best straight after the shoot.

Ask about feelings and frustrations, not specifications

'What do you want to feel looking at these in ten years' returns something you can shoot; 'what poses do you want' returns silence. Asking a student what frustrates them produces a curriculum.

Describe usage the way a contract would

'For my portfolio' and 'for a client campaign' are different permissions. Name the intended use precisely and record compensation even when it is none. Where a use is commercial or the subject is a minor, confirm what a release requires.

What is the difference between a client intake form and a shoot questionnaire?

The intake is the operational one, sent as soon as a session is booked: contact details, shoot type, date, location and how many people are being photographed. The questionnaire is the creative one, sent closer to the day, dealing in wardrobe, mood boards, must-have shots and the reason the photographs are being taken. Photographers who shoot one type of session often merge them; anyone shooting several usually keeps them apart.

Which answers let you quote a photography package without a call?

Type of photography, the preferred date and location, roughly how many hours of coverage are needed, whether albums or prints are wanted, and a budget range as a dropdown. Ask for approximate hours rather than an exact number, because clients rarely know. Make the free-text event details field required — a client who writes a paragraph is usually a client who is serious.

Can clients share a Pinterest board or upload reference images?

Both. An upload field takes reference images directly, up to 2 MB each, and a short text field takes a Pinterest board or a gallery. Contest entrants submitting full-resolution frames will exceed 2 MB, so point those at a link. Keep the mood board question optional: clients who have one will share it enthusiastically, and the rest can describe the look in words. Early access is free, with no ceiling on forms or responses.

Is a photo usage permission request the same as a model release?

No. This form is an intake that documents what is being asked for — the subject, when and where the photograph was taken, the intended use, the usage type and any compensation — and leaves you a dated record of the exchange. A formal model release is a separate document, and whether you need one depends on the use and the jurisdiction, particularly for commercial work or photographs of minors. Confirm your own obligations before licensing or publishing.
