The collection sorts by what you do with the answer. Scored surveys — NPS, customer and employee satisfaction, pulse polls, training and usability — repeat the same rating questions on a cadence so the number reads as a series. Votes and preference polls put a defined shortlist in front of people and ask them to choose. Open capture forms — feature requests, testimonials, waitlist signups, referral and ambassador applications — exist for the words people write, not for a score.

All face the same difficulty: getting an honest answer from someone with no particular reason to give you one. That is why so many of these templates pair a rating with an open follow-up and leave the name field optional.

## What is a survey form?

A survey form is a structured set of questions put to a defined group — customers, employees, attendees, residents — in a way that makes the answers comparable. Ten people describing their experience by email gives you ten anecdotes; ten people rating the same five dimensions on the same 1–5 scale gives you a distribution you can watch move next quarter.

Most mix three kinds of question. Closed scales produce the number: 0–10 for net promoter score, 1–5 for satisfaction dimensions, support or oppose or abstain for a community vote. Closed choices produce the ranking — a checkbox list of brand attributes or session topics, a single forced-choice radio. Open text produces the explanation, and that is usually where the useful material sits. A feedback form is the same instrument aimed at one interaction rather than a whole population, which is why several of these templates work as either.

## Why use survey & feedback form templates

A score only means something as a series

A net promoter score of 32 is inert; moving from 18 to 32 over two quarters is a finding. That takes identical questions each round, which is why these templates carry a run identifier — event name and date, product or training name — so rounds collect in one filterable table.

The rating says what, the open field says why

A six out of ten is unactionable on its own. Every scored template pairs its scale with a reason field — the main reason for your score, what could be improved, which topics needed more coverage — and that sentence is what makes a detractor recoverable.

Anonymity is a design decision, not a default

Employee surveys, opinion polls and pulse checks depend on the name field being optional and visibly so. Testimonial submissions and ambassador applications need the opposite: a real name, company and job title, and explicit permission for each channel you will publish on.

Segmentation turns an average into a diagnosis

Department and tenure on an employee survey, plan tier on a feature vote, acquisition channel on a brand awareness study. An overall satisfaction of 3.8 says nothing; 4.4 in one department against 2.9 in another is a decision.

A vote forces a priority that open feedback never will

Wishlists sprawl because nothing costs anything to add to them. The vote templates answer that with two stages — a capped multi-select for breadth, then one forced-choice question for the thing that matters most — which turns a list into a ranking.

## How to create your own

    Decide what the form has to produce — a tracked score, a ranked shortlist or quotable words — because that sets whether you need a fixed 0–10 scale, a capped checkbox list or a guided open field.

    Type the preamble above the first question: who is asking, how long it takes, whether responses are anonymous and what will be done with them. On an employee poll or a community vote it does more for answer quality than the questions below it.

    Open with the questions that produce that result — the 0–10 recommendation or the rating dimensions — and push name, email, department and tenure to the bottom, optional wherever honesty matters more than follow-up.

    Pair each scale with a follow-up and use conditional logic to make it specific: branch a 0–6 score into 'what went wrong', a 9–10 into a testimonial or referral ask, and give every rating dimension a not-applicable path so nobody scores a speaker your event did not have.

## Getting better responses

Fire it on the interaction, not the calendar

Post-event surveys go out within 24 hours while impressions are intact. A training evaluation belongs on the closing slide, before the room empties. Product feedback wants two to three weeks after delivery for physical goods, and roughly a month for software.

Let the format set the question count

NPS works because it is one rating and one reason; bolting five more questions on undermines what made it worth sending. A pulse poll sheds respondents above about five questions, and a post-event survey has roughly ten questions of goodwill to spend.

Do not write the answer into the question

A neutral attribute checklist beats asking whether people agree your brand is innovative. Community proposals need factual wording rather than advocacy language. In a preference poll, write every option with equal care, or the better-written option wins on prose alone.

Close the loop in public

A short summary of what came back and what changed as a result is the strongest thing you can do for the next round's response rate. On employee polls, community votes and feature requests alike, people stop answering when it leads nowhere.

## Frequently asked questions

How many responses do I need for a survey to be meaningful?

It depends on what the survey is for. If you are tracking a score over time, consistency of method matters more than volume — the same questions, the same trigger and the same audience each round will surface a real movement even at modest numbers. For a one-off vote you want enough responses that no single vocal group decides the outcome, and for open-ended feedback the themes usually start repeating well before you have exhausted your list.

What counts as a good Net Promoter Score?

NPS runs from -100 to +100 and is calculated as the percentage of promoters (9–10) minus the percentage of detractors (0–6), with passives (7–8) left out of the arithmetic. Anything above zero means you have more promoters than detractors. Published benchmarks vary so widely between industries that your own previous score is the more useful comparison, which is why running the survey on a fixed cadence matters more than hitting any particular number.

How do I make a survey anonymous?

Leave the name and email fields out, or mark them clearly optional and say so in the form's opening paragraph. Then look at your segmentation fields: department alone is usually safe, but department plus tenure plus location can single out one person on a small team, so keep the categories coarse. For sensitive internal surveys you can also switch on end-to-end encryption and host the responses in the EU.

What is the difference between a survey and a poll?

A poll asks one or two questions about a single decision and is usually run once — which session topics to schedule, which of four options the team backs, whether residents support a proposal. A survey asks a set of questions across several dimensions and is normally repeated so results can be compared. Pulse polls sit between the two: survey-style rating questions, poll-style brevity, run on a cadence.

How do I follow up with people who leave a low score?

Add an optional email field and use conditional logic to show it only to low scorers, with a line explaining that you would like to put things right. Every submission can fire a real-time webhook or a Zapier action, so a detractor score raises an alert in your team's chat or creates a task the moment it lands. That runs one way only: the submission triggers the action in the other app, it does not read anything back.
