# Plumbing Quote Request Form Template

A plumbing enquiry is two questions in one: what will this cost, and how fast do you need to be here. This form answers both. It asks which fixture or system is affected, what the symptom is, whether water is running right now and whether the stop valve has been closed, then adds property age, occupancy and a link for photos. There is no budget field, because a plumbing job is priced from the fault and the access, not from a spending band.

## About this template

A plumbing quote request is not a general service enquiry with the word plumbing on it. Two jobs on the same street, both described as "a leak", can differ by an order of magnitude in price and by two days in urgency, and the fields that separate them are specific: which part of the system is affected, whether water is escaping right now, whether the supply has been isolated, and how old the pipework is. A generic quote form asks for a service type and a budget, and neither answer tells a plumber anything.

This template is built around the stop valve question. If water is running into a property and nobody can turn it off, the response is a dispatch decision that has to be made in minutes, and it does not wait for a quote. If the supply is already isolated, the same fault becomes a scheduled repair that can be priced properly and booked for a convenient day. Asking whether the water is off, and offering "the valve is stuck" and "I do not know where it is" as real answers, is what makes the difference visible in the inbox instead of on the phone twenty minutes later.

The rest of the form supports the estimate. Fixture and symptom are separate questions because they are separate facts. Property age stands in for pipe material and fitting type. Occupancy decides whether the water can be left off overnight and whether the work can run out of hours. Access notes catch the loft hatch and the parking. And a link field for photos or a short video is the single highest-value optional field on the form, because a plumber can often diagnose from ten seconds of footage what would otherwise take a site visit. Every form built with formformform is hosted in the EU and handled under GDPR by default, and you can turn on optional end-to-end encryption for your own copy if you want submissions sealed to your key before they are stored.

## 12 form ideas you can build with this template

### Emergency Plumber Callout Form

Out-of-hours enquiry form where the live leak and stop valve answers drive an immediate phone triage rather than a written quote.

### Leak Detection Enquiry Form

For firms that trace hidden leaks: the symptom, water damage and property age fields together indicate whether tracing equipment is needed before any repair is priced.

### Blocked Drain Quote Request

Drainage-only variant, keeping the drains and soil stack options and cutting the heating ones, with property age standing in for pipe material.

### Boiler and Hot Water Repair Request

Narrowed to water heater, cylinder and heating faults, with the no-hot-water symptom and occupancy driving how fast an engineer is sent.

### Bathroom Installation Quote Request

Planned-work variant where the new installation option, property age and access notes replace the emergency triage as the pricing basis.

### Landlord Repair Reporting Form

Given to tenants by a landlord or agent, using the relationship field to route the job for authorisation before an engineer is booked.

### Letting Agent Maintenance Request

Managed-portfolio version where property address and occupancy identify the unit and whether the tenant will be there to give access.

### Commercial Premises Plumbing Callout

For shops, cafes and offices, where occupancy means trading hours and the water being off can close the premises.

### Frozen or Burst Pipe Report Form

Winter variant that leads with the live leak and stop valve questions and treats everything else as secondary.

### Water Pressure Complaint Form

For no-water and low-pressure calls, where the mains supply option, the property age answer and the free-text description shape the first visit.

### New Build Snagging Plumbing Report

Used during a defects period, where property age is known and the emphasis moves to fixture, symptom and photographic evidence.

### Facilities Water Fault Log

Internal form for a facilities team, where the same fields become a maintenance log entry and the stop valve answer triggers the building escalation.

## What's included

A stop valve question that separates a dispatch from a quote, with the four answers people actually give

A live leak question asked separately from urgency, so a customer's sense of panic does not have to stand in for the facts

Fixture or system asked apart from symptom, so the van is loaded from one answer and the diagnosis starts from the other

Property or pipework age, because what is behind the panel decides fittings, spares and labour

Occupancy captured for out-of-hours work and access, not as an afterthought in a notes box

A photo or video link field, with instructions for adding a real Upload field in the editor

No budget field at all, by design

Free, with unlimited submissions

## How to create a plumbing quote request form

1. 1

Click "Use this template" to load the plumbing quote request form into your free formformform account.

2. 2

Rewrite the intro paragraph with your real emergency number and your real response times. The urgency field records what the customer is asking for; only you can say what you actually offer.

3. 3

Trim the fixture and problem dropdowns to the work you do. A drainage-only firm should cut most of the heating options; a heating engineer should cut the drains.

4. 4

Set up an email notification for new submissions — or push them into Slack through the published Zapier trigger — and treat any submission where the water is still on as a call, not an email.

5. 5

Add conditional logic in the editor if you want the property questions to appear only for planned work. The editor supports branching; this template ships flat so it works the moment you clone it.

6. 6

Add an Upload field next to the photo link field if you would rather customers attach images directly. formformform supports uploads on any form, up to 2 MB per file, and encrypted on encrypted forms.

7. 7

Embed the form on your emergency or contact page, and put the direct link in your voicemail greeting so out-of-hours callers have somewhere to go.

## Best practices for your plumbing quote request form

Keep the stop valve question required. It is the one field that changes what happens in the next hour, and making it optional quietly turns this back into a generic enquiry form.

Ask whether water is running separately from how urgent it feels. People under-report emergencies out of politeness and over-report them out of panic. Two questions gets you the fact and the expectation.

Put your emergency phone number above the form, not below it. Anyone with a live flood should be calling, and a form that pretends otherwise costs you a bad review you did not earn.

Do not add a budget field. It is the default reflex on a quote form and it is wrong here: a customer cannot price a fault they cannot see, and a plumber cannot discount one. Ask what is broken instead.

Treat property age as a real estimating input. Pre-war pipework, mixed copper and plastic, and obsolete fittings are where a two-hour job becomes a day, and this dropdown is the cheapest early warning you will get.

Chase the photo link. A short video of a running leak is worth more than three paragraphs of description, and asking for it in the confirmation email as well as on the form roughly doubles how often you get one.

Never ask for key safe or alarm codes on a form. The access notes field says so explicitly. Take those on the phone at booking, and keep them out of your submission archive.

Give tenants a route that does not dead-end. The relationship field lets a tenant identify themselves; have a standard reply that loops in the landlord or agent rather than leaving the enquiry unanswered.

## Frequently asked questions

Is this plumbing quote form free?

Yes. Clone it, customise it, and collect unlimited submissions at no cost, with no per-submission fees.

Why is there no budget field?

Because it would not help either side. A homeowner cannot put a number on a fault they cannot see, and a plumber cannot quote from a spending band. The fields that actually determine the price are on the form: the fixture, the symptom, whether water is escaping, the age of the pipework and the access.

Why does the form ask whether the water is shut off?

Because it is the question that decides whether the enquiry is a quote or a dispatch. If water is still running into a property, the job needs a phone call within minutes. If the supply is already isolated, the same fault can be scheduled and priced properly.

Can customers attach photos of the leak?

This template ships with a link field for photos or video, because templates cannot arrive with an upload field pre-built. Add an Upload field to your copy in the form editor if you want direct attachments. formformform supports file uploads on any form, up to 2 MB per file, and they are encrypted on forms with end-to-end encryption switched on.

Should the phone number be required?

On this template it is, because urgent plumbing work is dispatched by phone and an email-only enquiry can sit unread. If you quote purely by email, edit the field and make it optional.

Can I use this for commercial plumbing enquiries?

Yes. Property type and the relationship field both include commercial options, and the occupancy question covers premises open to the public. For larger contracts you may want to add fields for site contact and permitted working hours.

How do I handle emergencies that arrive through the form anyway?

Set up an instant notification and build a triage rule around two fields: water running plus stop valve not closed means call, not email. Some firms also add a hidden field to tag submissions from their emergency landing page.

Is submission data stored securely?

Every form is hosted in the EU, with TLS in transit and encryption at rest, and submissions are handled under GDPR by default. You can also switch on optional end-to-end encryption for your own copy of this form, which seals each answer in the customer's browser before it is stored.
