## About this template

A privacy-first patient intake form asks only for what a first appointment genuinely requires, instead of defaulting to every field a general intake form has room for. Most templates collect insurance details, a full medical-conditions checklist and several required identifiers before a patient has even been seen — useful for a general practice's front desk, but more than a first visit always needs. This template starts from the other direction: what does the person on the other end actually have to know before the appointment, and what can wait.

The fields reflect that. Reason for appointment and a privacy notice acknowledgement are the only fields required beyond basic contact information. Date of birth ships optional, with a field description for practices that use it as a second identifier and want to switch it back to required. Emergency contact is a single optional field rather than three separate required ones, and current medication, allergies, and accessibility or communication needs are all optional — asked for, not demanded.
