# Satisfaction Pulse Poll Template

Catch burnout and disengagement before they become crises. This satisfaction pulse poll takes about two minutes and covers seven scored dimensions of team health — from workload and psychological safety to whether people have what they need to do good work — plus two open reflections on what to keep and what to change. ## About this template

A satisfaction pulse poll is a lightweight alternative to the traditional annual employee engagement survey. Rather than running one comprehensive survey per year and reacting to the results months later, a pulse poll runs on a recurring cadence — weekly, biweekly, or monthly — and gives managers and HR teams an ongoing signal that they can act on in real time.

This template scores seven questions across six themes — overall satisfaction, whether people understand their current priorities, whether they have what they need to do good work, workload manageability, psychological safety, and manager and team support — plus two open fields asking what to keep doing and what to change, and an optional opt-in if someone is open to a follow-up. Psychological safety in particular tends to move before overall satisfaction does: a team that stops feeling safe raising problems usually shows it here well before it shows up in an exit interview.

Every identity field in this template is optional — name, department, and the follow-up email are never required, and the form can be filled out and submitted without any of them. That's different from saying a response is untraceable: like every formformform submission, a pulse response is stored with the respondent's IP address and browser information, visible to whoever owns the form and included in CSV exports. If you want the poll to function as a genuine anonymous channel, share it as one link to the whole team rather than individualized links, and don't make the optional fields required. formformform makes the operational side simple either way — once the form is set up, re-sharing the link each period via Slack, email, or your team's intranet takes seconds.

## How to create a satisfaction pulse poll

1. 1

Click "Use this template" to open the satisfaction pulse poll in your formformform account.

2. 2

Adjust the phrasing on the rating questions to reference your specific period — "this week," "this month," or "this quarter" — to match your intended cadence.

3. 3

Customize the department dropdown to match your actual org chart, and remove it entirely for any team small enough that naming a department would identify who answered.

4. 4

If you want people to be able to opt into a follow-up, add a conditional rule in the editor so the email field only appears after someone answers "Yes" — the template ships both fields always visible, since conditional logic isn't something a template can carry automatically.

5. 5

Set your notification email and decide whether to review results individually or in aggregate with your HR team.

6. 6

Share the link via Slack, email, or your team portal at the start of each period, and set a reminder for anyone who hasn't responded.

## Best practices for your satisfaction pulse poll

Run it on a fixed cadence

consistency is what makes pulse data valuable. A monthly pulse sent on the first Monday of each month becomes a habit employees expect and complete without friction.

Keep the rating questions tight

seven scored questions plus two short open fields and a follow-up opt-in is close to the ceiling for a recurring check-in. Past that, completion rates drop fast enough that trend data gets noisier, not clearer.

Act on the results visibly

share a one-paragraph summary with the team each period. When employees see that low scores triggered a real change, future completion rates improve significantly.

Don't panic over single-period dips

one bad week doesn't indicate a cultural crisis. Look for sustained trends across three or more periods before escalating concerns.

Use the department breakdown to find pockets of stress, but drop the field for small teams

a department with a 3.2 average while company-wide is 4.1 needs attention, but on a two-person team that same breakdown just names who answered.

Read every open comment

quantitative scores tell you where to look; the "keep doing" and "change" fields tell you what's actually happening. Never skip either one, even when scores look healthy.

Don't promise untraceable anonymity

say plainly that names and departments are optional, not that the platform can't see who responded. formformform stores the respondent's IP address and browser information on every submission the same as any other form; the honest promise is that nobody has to identify themselves, not that the system can't.

Set expectations on the follow-up email

tell people to only fill it in if they picked "Yes" above, and if you want it to actually hide until then, add that rule yourself in the editor's conditional logic.

## Frequently asked questions

How often should I run a satisfaction pulse poll?

Monthly is the most common cadence and balances signal quality with respondent fatigue. Fast-moving or high-stress teams may benefit from biweekly pulses; stable, smaller teams may find quarterly sufficient.

What's the difference between a pulse poll and an annual engagement survey?

An annual engagement survey is comprehensive and infrequent — it's good for deep analysis but produces results too slowly to prevent attrition. A pulse poll is short and frequent — it catches issues in time to act on them.

How do I get employees to actually fill it out?

Keep it short (under two minutes), run it on a predictable cadence, and visibly respond to the results. Employees complete pulse polls when they believe their answers lead to action.

Are responses truly anonymous?

No response requires a name, department, or email — all three are optional and can be left blank. That said, like any form submission, a pulse response is stored with the respondent's IP address and browser information, visible to the form owner and included in CSV exports, so if you need the channel to function as anonymous, share one link with the whole group rather than individualized links and don't make the optional fields required.

Can I track satisfaction trends over time?

Yes. Export each period's results to CSV and maintain a running spreadsheet. Plot the average score for each dimension over time to see trends across the organization or by department.

What should I do when scores drop significantly?

Treat a significant drop — more than half a point on the 5-point scale — as a signal to investigate. Read the open comments first, then schedule brief one-on-one conversations with team leads to understand what changed.

Why does this template ask about psychological safety?

Whether people feel safe raising problems tends to move before overall satisfaction scores do, so it works as an early-warning signal rather than a nice-to-have. Watching it alongside workload and support gives a fuller picture of where a team is heading, not just where it is today.

Can I make the follow-up email field only show up when someone opts in?

Not out of the box — every field is always visible when you first use the template. If you want the email field to appear only after someone answers "Yes" to the follow-up question, add a conditional logic rule to that field in the form editor after cloning the template.
