Form templates for remote & distributed teams
Form templates for teams that do not share a building: inbound interest from remote developers, designers and support staff, morale pulse checks, home VPN access requests, and the addendum that records which state someone now works from. Each one arrives built, so the work is cutting the questions that only made sense in an office.
With no hallway, anything a manager would have noticed in a room has to be asked for outright. The monthly morale check exists because isolation does not announce itself; the toolkit poll exists because a bad chair is invisible on video.
The rest is logistics spread across time zones: shift cover on a support inquiry, seasonal VPN access that has to come off when the contract does, equipment policy on an employment addendum.

What remote & distributed teams usually collect
Remote role forms turn on when someone is actually reachable. Sales candidates give time zone coverage, support candidates give shift availability, and writers and analysts state how much of the job they expect to run async.
Designers attach a portfolio or link to one, writers attach samples, developers paste a GitHub URL. Whatever they send lands on the submission itself, so whoever reviews it reads the work next to the tool proficiency and availability answers.
A work-from-home VPN request asks for the router model and the ISP, because the person fixing it is on chat, not walking to the desk. The toolkit poll asks whether the kit the company sent is actually doing the job, and the satisfaction survey asks about the home office setup and the communication tools around it.
A remote work agreement addendum starts with the state or country the employee will actually sit in, then equipment policy and expense reimbursement terms. Location drives payroll and employment law, so it is a required field.

How to set one up
- 1
Open the Remote Work Opportunity Inquiry template and pick one role to start with. The developer version asking for tech stack and GitHub reads nothing like the support version asking about shift cover.
- 2
Branch on role with conditional logic so one form serves all of them: designers get a portfolio upload, accountants get software proficiency, sales candidates get quota history and time zone coverage.
- 3
On the internal side, make the contract end date a required field on a seasonal remote access request. The form cannot revoke anything, but every request then arrives carrying the date its access was meant to stop, rather than that date being reconstructed months later.
- 4
Send pulse checks down the channel the team already reads. There is no noticeboard to pin a QR code to, and a survey posted at nine in the morning for one half of the team arrives in the middle of the night for the other.
- 5
Let each submission do the handoff, since there is nobody to walk it over to. A webhook fires in real time, or a Zapier action does the same job without code: a remote access request opens a ticket, a role inquiry lands in the hiring tool. That direction is the only one on offer — a submission triggers the action, and nothing is ever read back out of the connected app into the form.
Frequently asked questions
How do you measure morale on a team that never meets in person?
Ask on a schedule, and keep it short. A monthly pulse of four or five questions catches the drift that a corridor conversation would have surfaced in an office — isolation, workload, whether someone has spoken to a colleague outside their squad this week. Responses can be filtered and exported, so you compare this month against last rather than reading each one cold.
Can remote candidates upload a portfolio or writing samples?
Yes, up to 2 MB a file. A designer's portfolio PDF or a writer's samples come in attached to the submission — one upload field per file — so nobody is chasing a link that has gone dead since the application went in. You preview or download them beside the tool proficiency and availability answers. Anything heavier, a showreel or a full case-study deck, is better asked for as a URL.
How do we stop seasonal remote workers keeping VPN access after their contract ends?
Capture the expiry at request time. Make the contract end date and the approver required fields on the access request, then filter responses by that date to see what should already have been withdrawn. Revoking still happens in your identity provider — the form's job is to make sure nobody is guessing when access was meant to stop.
Our team is spread across the EU — where is the data stored?
EU hosting is available, and everything is encrypted at rest and in transit. The service is fully GDPR compliant, which matters more than usual when your employees are resident in several member states at once. There is no SOC 2 report and no ISO 27001 certificate, so if a client security review demands one, say so early.
Can an employee sign a remote work agreement addendum on the form?
No. A form can record typed agreement — a tick-box and their name against the wording shown — which is enough to gather the details your legal team needs: the work state, the equipment policy, the reimbursement terms. The executed document still gets drawn up and signed elsewhere. There are no e-signature fields.
