Form templates for hotels & hospitality

Hotels, resorts, hostels, B&Bs and short-term rental hosts run on paperwork that sits either side of the booking engine and never inside it. That is what this page is for: guest feedback, pre-arrival preference cards, group room blocks, concierge requests and room maintenance reports, ready to edit in formformform.

Each stage of a stay wants a different form. Pillow menu and bed configuration before arrival; an airport transfer or a fine-dining booking during; NPS the morning after checkout, before the guest writes a review anywhere else. Group business runs slower: a wedding block or a medical conference is quoted months ahead.

The back of house is on the same account: banquet cook applications for an event-driven roster, vendor enquiries carrying insurance details, a renovation punch list closed room by room. Free during early access.

What hotels & hospitality usually collect

Preferences before arrival

Pillow firmness, bed configuration, minibar brands, dietary restrictions for the welcome amenity. Accessible room requests go further — mobility device dimensions, grab bar placement, shower chair height — because housekeeping stages the room before the guest ever sees it.

Room block logistics

A room block enquiry lives or dies on billing: master account or room-to-room. After that come attendee count, breakout and AV needs, per diem rate for government training groups, equipment storage and early breakfast hours for a travelling team.

Room number and department

Complaint and service forms need the room number, the dates and the department involved, so follow-up reaches the right head of department. Naming the front desk agent turns a survey into recognition rather than a score with nobody attached.

Concierge and activity requests

Concierge and activity requests are logistics with a safety question attached: flight number and terminal for a transfer, certification level for a dive, swim ability and allergies for the kids club, therapist preference and skin sensitivity for a treatment.

How to set one up

  1. 1

Start from the Hotel Guest Feedback Form template. It opens as a document, so you edit the questions by typing over them and add fields inline wherever the property needs one.

  2. 2

Branch it with conditional logic: ask the stay type first, then show spa questions only to spa guests and meeting-room questions only to the corporate block. One form covers the whole property.

  3. 3

Publish it three ways from one form: a QR code on the in-room compendium and lift lobby, a link in the post-checkout email, and a two-line embed on the property site.

  4. 4

Route submissions with a webhook or Zapier so maintenance reports reach engineering and group enquiries reach sales. Connections run in one direction — the submission fires the action, nothing is read back.

  5. 5

Before high season, stand up the rest of the stay in the same account: Hotel Room Preference Form for pre-arrival, Concierge Service Request Form during, Hotel Room Maintenance Request Form for whatever breaks.

Frequently asked questions

Does formformform connect to my PMS or channel manager?

Only through webhooks and Zapier, which cover 60+ apps, and only outward. Each submission fires an action — a group enquiry opens a deal in the CRM, a maintenance report creates a task. Nothing flows the other way: rates, availability and reservations stay in the PMS, and there is no two-way sync.

How do I collect guest feedback before the review goes online?

Timing is the whole trick. Send the NPS or post-stay survey the morning after checkout, while the stay is fresh and before the guest sits down to write a public review. A QR code in the compendium catches the ones who prefer to say it on the spot, and responses can be filtered by room or date.

Can I ask for passport or ID details on a pre-arrival form?

Yes, on an end-to-end encrypted form. A digital registration card asks for passport or ID number, nationality and address — the details many properties are still copying off paper at the desk. Data is end-to-end encrypted, encrypted at rest and in transit, and can be hosted in the EU under GDPR.

Can guests send photos of a maintenance problem in the room?

Yes. File Upload is a field type, so the guest or the room attendant attaches the shot from their phone — the dripping tap, the stained carpet, the safe that will not open — and engineering opens it from the submission beside the room number, the issue type and whether housekeeping may enter while the guest is out. Each file is capped at 2 MB.

Can I use the same forms for seasonal and banquet hiring?

Yes. Kitchen Staff Application Form and Fitness Instructor Application Form sit in the same account as the guest-facing ones, and a banquet cook application can ask about large-batch cooking, buffet setup and availability for an event-driven roster. Responses stay searchable, so when the season turns you reopen the same form rather than rebuilding it.
