Prayer and pastoral requests

The request itself, and how far it may travel: leaders only, the small group, or a public prayer wall. Prison ministry and children's ministry versions route instead to a chaplain team or a class teacher.

Membership and transfer details

Household members and ages, baptism preference, the sending church's pastor on a denominational transfer, and which campus a multi-site applicant wants to call home. International congregations add country of origin and primary language.

Volunteer and team screening

Which service time, which age group, and whatever the role genuinely requires: CPR certification for the nursery, food handling for the pantry, passport status and trade skills for a build trip, background check consent wherever children are involved.

Pledges and giving records

Pledge amount, the fund it belongs to — building, church plant, a sponsored seminary student — and whether it comes weekly, monthly or as one payment. Annual receipt requests need the giving year and the name on the record.

How to set one up

    Start from the Ministry Volunteer Sign-Up template, or Prayer Request Form if that is the nearer job, and reword the questions the way your congregation would say them.

Frequently asked questions

Can we collect tithes or pledge payments through the form?

Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.

How do we keep prayer requests private?

Ask on the form itself whether a request goes to leaders only, to the small group, or onto the public prayer list. For the sensitive end — a benevolence fund application, a restoration and re-membership request, a counselling intake — use an end-to-end encrypted form. EU hosting is available and the service is GDPR compliant.

Can volunteers send us their background check or CPR certificate?

Yes. File Upload is a field type, so a nursery volunteer attaches the CPR card and a pantry volunteer the food handling certificate, and you download or preview it from the submission. Ask for the certificate number and expiry date as plain fields alongside it, so you can search for what has lapsed without opening every file. On an end-to-end encrypted form the attachment is stored encrypted and decrypted in your browser. Keep the background check consent checkbox on the form for any nursery or children's ministry role.

Can one form handle a whole family at once?

Yes. Conditional logic can ask how many children are being registered, then reveal a short block of questions for each — name, age, allergies, class — which is how family membership applications and VBS sign-ups usually work. Keep each block brief, or a parent with four children gives up halfway.

Is it free for a small congregation?

Everything is free during early access: unlimited forms, unlimited responses. A Vacation Bible School registration that takes three hundred families costs the same as a prayer form that takes six, and there is no per-response cap to watch during Holy Week or the autumn pledge drive.
