Form templates for senior care & assisted living

Senior care runs on forms that someone other than the resident fills in — an adult daughter booking a tour, a discharge planner sending a referral, an aide logging visit hours at the end of a shift. This page gathers the templates that fit assisted living, independent living, home care and community aging services.

The work is split across shifts and buildings. Activity directors book the community room, maintenance logs call-button and grab-rail faults, care coordinators match aide availability to client zones, and the front desk screens visitors before they reach a resident's floor.

Every template here opens in a document editor you can type into. Rename the care levels, branch the questions so a family member sees different fields to a resident, then share by link, QR code or an embed on the community site.

What senior care & assisted living usually collect

Mobility and fall risk

Mobility aids, fall history, balance concerns and whether an exercise class should be seated or standing. The same answers decide wheelchair lift requirements for a group outing and how much transfer help a chair yoga session needs.

Who is answering

Relationship to the resident, the family contact who should be called first, and whether a power of attorney is in place. Tour sign-ins and dining enrolments are usually completed by a son or daughter, not the resident.

Faults in the building

Apartment number, wing and floor, plus what actually failed: a call button, an accessible bathroom fixture, a grab rail, the HVAC. Move-in condition reports and repair requests both need the unit named the way maintenance names it.

Hours, zones and shifts

Aide availability by geographic zone and client continuity preference, visit hours with the client names served, and companion volunteer logs that carry mobility or communication notes back to the care team.

How to set one up

  1. 1

Start from a template that already matches the moment — Senior Living Open House Registration for a tour day, Elder Care Facility Visitor Screening for the front desk — and edit the wording inline.

  2. 2

Add conditional logic so the questions follow the answer: a family member filling in for a resident sees the relationship and contact fields; a resident answering directly skips them.

  3. 3

Turn on end-to-end encryption for anything sensitive — elder abuse crisis intake, the cognitive-status question on a power of attorney enquiry, housing assistance income details — before you publish.

  4. 4

Share it where the person already is: a QR code by the sign-in book, a link in the family newsletter, an embed on the community's enquiry page.

  5. 5

Point a webhook at the tools you already run, so a care referral reaches the coordinator and a visitor screening lands with the duty nurse without anyone rekeying it.

Frequently asked questions

Is formformform HIPAA compliant?

No. There is no HIPAA compliance, and no SOC 2 or ISO 27001 certification either. The claims that hold up are GDPR, EU hosting, encryption at rest and in transit, and end-to-end encrypted forms. That covers a visitor screening at the front door or a companion volunteer log; a resident's clinical record belongs in the system your clinical team is licensed to run.

Can a family member or power of attorney complete a form for a resident?

Yes, and that is the normal case for tour sign-ins, dining enrolments and discharge notices. Ask who is completing the form first, then branch: a daughter or POA gets the relationship, contact and authority questions, while a resident answering alone goes straight to the rest.

Can families send us a care plan or power of attorney document?

Yes. File Upload is a field type, so an adult child attaches the scan alongside the typed answers — the signed power of attorney, a hospital discharge summary, the current medication list — and you download or preview each file from the submission. Files are capped at 2 MB each, so a long care plan may arrive as a few. On an encrypted form the attachment is encrypted with the answers and decrypts only in your browser, which is worth turning on before an elder law POA enquiry asks about the principal's cognitive status.

Can the open house form book the tour slot as well?

No. There is no calendar or scheduling built in. Collect the care level needed, preferred days and the lunch RSVP on the registration form, then confirm the slot from whatever diary the community keeps. A webhook can push each new registration into it as it arrives.

How do home health aides send in availability and visit hours?

One link per pay period, opened on a phone between visits. The availability form takes open slots, client continuity preferences and the zones they can cover; the hours form takes the date, client names served and coordinator confirmation. Filter and export the responses when payroll runs.
