How does a parent sign a waiver for their child online?

A form can record typed agreement: the guardian's name, the date, and a checkbox confirming they have read the risk acknowledgement and authorise emergency medical treatment. That is not a legally executed signature — there are no e-signature fields. If your league insurer or governing body wants a countersigned document, keep that step where it is and use the form for disclosure, emergency contacts and consent.

Can one form cover every age division from U6 to adult?

How do we collect proof of age, a sports physical or a coaching certificate?
