Form templates for hoas & community groups
Form templates for HOA and condo boards, residents' associations and community groups, ready to edit in formformform: architectural review applications, covenant concern reports, guest parking passes, clubhouse and tennis court bookings, agenda items for the next board meeting, and the vote on a rule change.
A board is volunteers doing this around a day job, and it turns over at the annual meeting. Forms have to outlast whoever built them: a covenant concern, a variance request and a tree removal all land in one searchable place, ready for the meeting packet.
The year has a shape: pool passes in spring, the summer social and block party, then budget, audit prep and elections in autumn. Assessment hardship requests carry household finances, so those belong on an encrypted form rather than an email thread to the treasurer.

What hoas & community groups usually collect
Architectural review applications want dimensions, materials, paint colour, the contractor's name and the dates work will run. The plan set, the elevation drawings and a photo of the paint chip attach to the same submission, so the committee reviews one record instead of an email thread. A surveyor's full drawing pack will be over the 2 MB ceiling on a single upload, so ask for a shared-drive link in a short text field instead.
Guest passes and resident permits need the plate, make and colour, the dates of the visit, and the unit the vehicle belongs to. Violation reports need the same details plus the spot and the time it was seen.
Clubhouse, pool and tennis court bookings start with proof of standing — address, unit number, sometimes whether assessments are current — then the date, hours, headcount and whether alcohol or a bouncy castle is involved.
A covenant concern report takes the address, the date and time, the rule involved, and what the resident saw — overgrown lawns, an unapproved shed, bins left out. Name and contact can be optional if your governing documents allow it.

How to set one up
- 1
Open the HOA Request Form template and type over it. The editor works like a document, so fields for the lot address, request type and description drop in as you write.
- 2
One form can carry every request type once you branch it with logic. Architectural review asks for contractor and dimensions; a variance asks which covenant and why; a tree removal asks for the replacement plan.
- 3
Put it where residents already look: embed it on the community website with two lines of HTML, pin the QR code to the clubhouse notice board, and drop the link into the monthly newsletter.
- 4
A webhook or a Zap moves each new request into wherever the board already works — the shared inbox, or the management company's task list. Traffic runs one way: a submission triggers the action in the other app, nothing comes back.
- 5
Before the annual meeting, put the rule change on a Community Vote Form and the assessment hardship request on an encrypted form, then export both when the minutes need writing.
Frequently asked questions
Can we run an HOA vote or board election through a form?
For a straw poll or a rule-change sentiment check, yes — the Community Vote Form collects support or opposition, and responses are searchable and exportable. For a binding ballot, read your CC&Rs or bylaws first: many require secret ballots, notice periods, quorum counts and proxy handling that a form will not enforce. Nothing counts the ballots for you — there are no calculated fields — so the tally happens in a spreadsheet after export.
How do homeowners send photos with an ARC application or violation report?
They attach them. The ARC application takes a File Upload field for the elevation drawings and the paint chip; the concern report takes one for the shot of the fence line or the boat on the driveway, which the homeowner usually has on their phone already. Each file arrives with the rest of the submission, and the board downloads or previews it from the response. Where the form is end-to-end encrypted, the files are sealed with the answers and open only in your browser. The ceiling is 2 MB per file, and a photo straight off a phone often lands over it, so put the limit in the field's description and ask for a link in a text field when a drawing pack is genuinely bigger.
Can residents report a covenant violation anonymously?
You decide. Leave name and contact off the form, or mark them optional, and the report arrives with only the address, date and description. Or keep them required and say why on the form: a submission is only ever read by whoever opens the responses, never posted anywhere, so the board can follow up while the reporter stays out of a conversation over the fence. Check what your governing documents say about anonymous complaints before you choose.
Can we collect assessment payments or set up a payment plan through the form?
Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.
Do we need a separate form for each amenity?
One reservation form can cover the clubhouse, the pool deck and the tennis court. Ask which amenity first, then show only what applies: court bookings need a member address and unit number as an identity check, clubhouse bookings need hours, headcount and a cleaning deposit acknowledgement. Responses stay in one list you can filter by date or by amenity.
