Frequently asked questions

Can members sign a liability waiver on the form?

A form can capture typed agreement — name, date, and a checkbox confirming the participant has read the terms — stored with a timestamp against the submission. It is not an e-signature product and there are no signature fields, so if your insurer requires a countersigned document, keep that part where it is and use the form for screening and contact details.

How do we collect a doctor's note or a physical therapy discharge summary?

Add a file upload field and the client attaches it with the rest of the intake — a discharge summary, an OB clearance note, a photo of the exercise sheet their physio wrote. You download or preview it from the submission, and on an encrypted form the file is encrypted alongside the answers. Files are capped at 2 MB each, so keep a short text field for the clearing clinician's name and date as well.

Is it safe to collect health screening answers on a form?

Everything is encrypted in transit and at rest, EU hosting is available, and forms are fully GDPR compliant. For a par-Q style screening or a cancellation citing a medical reason, turn on end-to-end encryption. We do not offer HIPAA compliance, so do not treat these submissions as a clinical record.

Can the form take payment for a membership or a challenge entry?

Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.

Can a cancellation form offer a freeze instead of cancelling?

Yes, with logic. When the reason is cost or injury, show the freeze option, the reduced-rate tier, or a consent checkbox for a win-back offer later; when it is relocation, ask about the sister location instead. The submission still records the member ID and notice date, so the front desk has what it needs either way.
