Can I take an insurance application through an online form?

You can take the information an application needs, and route it into your rater or agency management system. The form does not bind coverage and it is not an e-signature product — a checkbox captures typed agreement to terms, but the signed application still goes through the carrier's portal. Say so on the confirmation screen.

How do I collect health details and dates of birth safely?

Put the health quote and the underwriting pre-screen on an end-to-end encrypted form — dates of birth, tobacco use and self-reported conditions then decrypt only in your browser, and an attached document is encrypted the same way. Everything is encrypted at rest and in transit, EU hosting is available, and the service is GDPR compliant. No security certification or health-data compliance programme sits behind that, so if a carrier's contract asks for one, settle it before you route medical detail here.

Can one form handle personal lines and commercial?

Yes. Ask up front whether the quote is for a household or a business, then branch. Personal goes to vehicles, drivers and dwelling details; commercial goes to employee count, class codes and DOT number if there is a fleet. Neither side sees the other's questions, and both land in the same response list.
