Room-by-room enrolment detail

Infant registration asks for feeding schedule, formula or breastmilk, and sleep position. Toddler rooms need nap routine, comfort item and toilet-training status. Pre-K asks about kindergarten readiness, home language and IEP status.

Who may collect a child

A pickup list names both parents and up to five other adults. After-school programmes add whether a child may walk home or take the bus; nursery rooms release only to the adult who dropped the child off.

Health and immunisation records

State-required vaccination dates, allergy lists, medication routines, and any developmental evaluation or related services. A file upload field takes the photo of the immunisation card, so the typed dates and the document the paediatrician issued arrive on the same submission.

Frequently asked questions

Can a parent attach the immunisation record to the enrolment form?

Yes. File Upload is a field in the editor, so a parent photographs the card from their phone and it arrives attached to the enrolment, for you to download or preview from the submission. Files are capped at 2 MB each, so a long multi-page record is better sent as a link in a short text field. Ask for vaccine names and dates as typed fields as well — those are what you search when you need to know which children are due a booster.

Can one form enrol two children from the same family?

Yes. Ask how many children are starting, then use conditional logic to reveal a second set of child fields when the answer is two. Parent details, the pickup list and the billing contact stay entered once. Siblings heading to different rooms can still be routed to different questions.

Is it safe to put children's details in an online form?

Submissions travel encrypted and are stored encrypted, EU hosting is available, and forms carrying health or custody detail can be end-to-end encrypted so only your team can read them. The service is fully GDPR compliant. Your licensing file may still need a paper copy, and a form does not replace that.

How do parents find the waitlist form at an open house?
