Form templates for childcare & daycare
Childcare enrolment starts long before a first day: a waitlist entry, a room offer, an immunisation record, a list of adults allowed at the door. These templates cover that sequence for daycare centres, preschools, in-home providers and after-school programmes, and each one is yours to reword.
Demand arrives in waves. Infant spots — six weeks to twelve months — are the ones that run out first, summer programmes fill session by session, and before-and-after-school places depend on which school a family is tied to. A waitlist form that stays open between those points is how a place gets filled the week it opens.
Licensing adds the rest. Vaccination documentation is state-required for a licensed programme, and every child needs a named list of the adults allowed to collect them — details that change mid-year, long after the paper pack went home in a bag.

What childcare & daycare usually collect
Infant registration asks for feeding schedule, formula or breastmilk, and sleep position. Toddler rooms need nap routine, comfort item and toilet-training status. Pre-K asks about kindergarten readiness, home language and IEP status.
A pickup list names both parents and up to five other adults. After-school programmes add whether a child may walk home or take the bus; nursery rooms release only to the adult who dropped the child off.
State-required vaccination dates, allergy lists, medication routines, and any developmental evaluation or related services. A file upload field takes the photo of the immunisation card, so the typed dates and the document the paediatrician issued arrive on the same submission.
Applications ask for availability in real blocks — Friday and Saturday evenings, 2–6pm weekday pickups, full summer days — alongside CPR certification, comfort with infants, background check consent and contacts for families they have worked for.

How to set one up
- 1
Open the Daycare Enrollment Form template and rename its sections after your actual rooms — infants, toddlers, preschool, Pre-K — so a parent recognises the programme they were offered.
- 2
Add conditional logic on the child's age so infant questions, such as feeding schedule and sleep position, appear only for the under-ones, and toilet-training questions only for toddlers.
- 3
Publish the Childcare Waitlist Registration Form separately and keep it open year-round. Embed it on your site with two lines of HTML, and print its QR code for open house night.
- 4
Turn on end-to-end encryption for the form that carries immunisation dates and medical notes — attached records are encrypted with the rest of the answers and decrypt in your own browser — and choose EU hosting if your families ask where their children's data sits.
- 5
Point a webhook at your management system or Zapier so each submission creates a record in the tool you already use, then filter responses by room and start date when you plan staffing ratios.
Frequently asked questions
Can a parent attach the immunisation record to the enrolment form?
Yes. File Upload is a field in the editor, so a parent photographs the card from their phone and it arrives attached to the enrolment, for you to download or preview from the submission. Files are capped at 2 MB each, so a long multi-page record is better sent as a link in a short text field. Ask for vaccine names and dates as typed fields as well — those are what you search when you need to know which children are due a booster.
Can one form enrol two children from the same family?
Yes. Ask how many children are starting, then use conditional logic to reveal a second set of child fields when the answer is two. Parent details, the pickup list and the billing contact stay entered once. Siblings heading to different rooms can still be routed to different questions.
Is it safe to put children's details in an online form?
Submissions travel encrypted and are stored encrypted, EU hosting is available, and forms carrying health or custody detail can be end-to-end encrypted so only your team can read them. The service is fully GDPR compliant. Your licensing file may still need a paper copy, and a form does not replace that.
How do parents find the waitlist form at an open house?
Print the form's QR code on the tour handout and the room signs, so a parent can join the list before they reach the car park. The same form can sit on your site as an embedded block or a slide-in on the fees page. One link, one set of responses.
What does this cost for a small daycare?
Everything is free during early access — unlimited forms and unlimited responses. An enrolment run across every room, a year-round waitlist and a stack of sitter applications all sit under the same account without counting against a quota or a per-form limit.
