Can parents sign the camp activity waiver on the form?

A form can record that a parent read the waiver and ticked to agree, with a timestamp and their name against the submission. That is a documented acknowledgement, not a legally executed signature — formformform is not an e-signature product. Camps that need a witnessed signature usually keep that step where it is.
