Form templates for web designers & developers

Templates for the forms a web studio runs on: the project enquiry that qualifies a lead, the UX feedback round, the revision request, the contract questionnaire. Each opens in the formformform editor, where you type and fields drop in inline.

A restaurant site and a law firm site are not the same brief. One needs menu format, ordering and delivery partner links; the other needs practice-area pages, attorney bios and bar-compliance copy. One enquiry form covers both, branching after the first answer so nobody scrolls past a screen of questions written for somebody else's project.

Feedback and revisions run the other way. A written wireframe change request is dated, attributed and countable against the rounds in the contract, which a thread of screenshots never is.

What web designers & developers usually collect

Scope before the quote

Project type, page count, CMS preference and style references — plus the vertical specifics that decide everything: current platform and catalogue size for a shop, MLS integration and search filters for an estate agency.

Stack and handover terms

Tech stack and CMS requirements, integration needs, device targets and any existing design system. On the contract side, the questions that cause arguments later: who owns the hosting, which browsers are supported, what counts as a milestone.

Rated UX feedback

Step-by-step ratings rather than opinions: where a client's checkout loses people between cart, shipping, payment and confirmation; drop-off reasons in onboarding; empty-state confusion in search; keyboard and screen-reader notes from accessibility testers. Consent first, when the session is screen-recorded.

Change requests, itemised

Navigation restructures and section reordering on wireframes; screen-by-screen component states and accessibility fixes on app UI. One request per submission, so a revision round is a list you can price, not a chat log.

How to set one up

  1. 1

Web Design Project Inquiry is the template to start from. Strip it back to the work you actually take on — a studio doing restaurants and local businesses has no use for the MLS questions.

  2. 2

Set the first question to project type and let logic do the rest: a redesign path asks about the current platform and catalogue size, a new build asks about sitemap and launch date.

  3. 3

Paste the two-line embed into your own site, and into client staging builds when you run their feedback rounds. The script is async and under 8kb gzipped.

  4. 4

Wire the webhook to whatever tracks your work, so an enquiry lands as a ticket rather than an email you re-type into the tracker yourself. The REST API is there if you would rather poll responses instead.

  5. 5

Keep a second form for delivery: a revision request that clients use during the build, with responses filtered per project so the round count is on record at invoicing.

Frequently asked questions

Can I embed a form on a client site without touching their build?

Two lines of HTML, pasted anywhere markup goes — a template partial, a CMS embed block, a static page. Nothing to install and no build step to change. The script loads asynchronously and is under 8kb gzipped, so it will not undo a performance budget you spent a week defending.

How do clients send me Figma links, staging URLs and brand files?

Links, mostly. A short text field takes the Figma file, the staging address, the site you are replacing, the shared drive folder — which is where design work already lives. When something has to arrive as a file, an upload field handles it: the client attaches the logo pack or the old brand guide, and you download or preview it from the response. Anything heavy is still better sent as a link.

How do I stop revision requests turning into unpaid scope creep?

Route them through a revision request form instead of email. Every change arrives dated, attributed and separate, so the third round is visibly the third round. Responses can be filtered and searched per project, which gives you something to point at when a client remembers the request differently.

Can the form live on our own domain instead of a shared link?

Yes — forms can be shared on your own domain, which matters when the enquiry form sits on a client site you built and a third-party URL would break the illusion. The same form is also available as a link, QR code, popup, slide-in or inline block, so one build covers a landing page, a pitch deck and a staging site.

Can submissions land in my project tracker automatically?

Each submission fires a webhook the moment it lands, and there is a REST API if you would rather pull. For the tools you do not want to wire by hand, 60+ apps connect through Zapier — a new enquiry can open a Linear issue or add a Notion row with the answers already on it. It runs one way: the submission triggers the action, and nothing is written back into the form.
