Other people set the calendar. Public comment closes before the council meeting; a records request starts a statutory clock the day it lands; a special event permit wants a route map, closure times and an attendance estimate before the parade, the 5K or the holiday market is announced.

Service requests do not wait either — potholes after the freeze, storm drains before the next rainfall — and a resident reporting them is standing on the pavement with a phone, photo attached. Responses stay searchable and exportable, which is what the public record needs. Anything carrying ID type or clearance level belongs on an encrypted form.

What government & public sector usually collect

Site, scope and licence numbers

Permit applications live on specifics: deck dimensions and materials, panel count and inverter specs on a solar install, trunk diameter and species for a protected tree, asbestos survey status and the contractor's licence number.

Location precise enough to dispatch

A crew cannot be sent to 'near the park'. Pothole reports need the intersection, a light outage needs the pole number, a park request needs the play structure — and a photo of the dumped mattress.

Event footprint and closures

Special event permits ask for the shape of the thing: a 5K route map and closure timing, stage placement and amplified sound for an outdoor concert, vendor count and waste plan for a farmers market.

Vendor and visitor credentials

Contractor onboarding wants DUNS number, NAICS codes, SAM.gov registration and small business certification; a municipal bid adds DBE status, bonding capacity and prevailing wage attestation. The lobby desk wants ID type and issuing authority.

How to set one up

  1. 1

Open the Permit Application Form template and retype it the way your department words it. The editor behaves like a document — address, dimensions and contractor licence fields drop in as you type.

  2. 2

Add conditional logic so one permit form covers the whole counter: a solar application asks for inverter specs, an ADU asks for unit size, a demolition asks about asbestos.

  3. 3

Embed it on the department page with two lines of HTML, then put a QR code on the counter sign, the park noticeboard and the notice taped to the closed street.

  4. 4

Send each submission on by webhook or Zapier so a pothole report opens a work order for the right crew. The flow runs one way: a submission triggers the action, nothing syncs back.

  5. 5

Copy the pattern across the rest of the counter — Public Comment Form ahead of each council meeting, Public Records Request Form for the response clock, an encrypted Visitor Log for badged buildings.

Frequently asked questions

Can residents attach a photo to a service request?

Yes. File Upload is a field in the editor, so an illegal dumping report can carry a picture of what was left and where, and a graffiti report can show the surface and the content before an officer decides whether to prioritise it. You download or preview the file from the submission.

Can we collect permit fees through the form?

Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.

Is an online form enough for a public records request?

For intake, yes. The form takes the requester's details, the records sought, the address or date range, and how they want them delivered — a building permit file for one parcel, or council email on a named topic. Each submission is timestamped and searchable, which gives you a defensible log against the response deadline. Determinations and fee waivers stay in your existing process.

What compliance certifications do you hold?

A short list, and worth checking before a procurement file gets opened. The service is GDPR compliant, EU hosting is available, data is encrypted at rest and in transit, and end-to-end encryption can be turned on per form. There is no SOC 2, no ISO 27001 and no HIPAA compliance — if your security questionnaire requires one of those, this will not clear it.

Can a contractor sign the prevailing wage attestation on the form?

Not as a legal signature. There are no e-signature fields. A checkbox can record that the bidder agrees to the stated wage and DBE language, timestamped with the rest of the submission, which is enough to move the bid into review while your contracting officer verifies SAM.gov registration and bonding capacity. Execution stays with your existing procurement paperwork.
