Retail runs on a calendar the rest of business ignores. Holiday hiring opens months before the peak, which is why the toy store application asks about working with children and the clothing store one asks about weekend availability and folding. A staff pulse survey follows in December, while the trading is still on.

Then the counting. A bi-annual apparel stocktake is one submission per style, size and colour against the POS record; a pharmacy counts controlled stock daily with an approver named on the form. Publish either as a link or a QR code, free during early access.

What retail stores usually collect

Counts against the POS record

A stocktake submission is a style, a size, a colour and a number, filed one row per combination. Controlled substance counts add the dispensing-system figure and the supervisor who witnessed it.

Return and refund detail

Fashion returns turn on the style number, the size and whether the tags are still attached, because that decides whether the garment goes back on the rail. Order number and desired resolution keep the exchange moving.

Certifications and legal age

Hiring forms carry the checks the role genuinely needs: food handler certification and forklift experience for grocery, pharmacy technician certification, and an age question on the wine and spirits floor, where most US states set the bar at 21.

Department-level feedback

In-store surveys only earn their keep when they name the department and the dimension that matters there — fitting room comfort, prescription wait time, produce freshness, sales pressure on a furniture floor, aisle signage in home improvement.

How to set one up

  1. 1

Start from the In-Store Experience Feedback Form template and cut it down to one department: type over the questions in the editor, drop new fields inline, delete the ratings your floor does not run.

  2. 2

Branch the form with conditional logic: a shopper who came in for a return never sees the fitting room questions, and a low service rating opens the follow-up fields a manager can coach from.

  3. 3

One form can go out on three surfaces at once: a QR code printed on the receipt and on shelf talkers, a kiosk link on the tablet at the till, and a two-line embed on your store page.

  4. 4

Add a store location field to every form so responses from every branch land in one place, then filter and search submissions by site when the district manager wants a like-for-like read.

  5. 5

Send each submission where the decision happens: a webhook or Zapier connection drops vendor pitches into the buyer's inbox and reorder requests with central purchasing. Leave the encrypted forms out of that path — an integration carries no readable content off an encrypted form, so loss prevention opens theft statements in the responses list instead.

Frequently asked questions

Does it sync with my POS or inventory system?

Not two ways. Submissions can fire a webhook or one of 60+ Zapier connections, so a stocktake row or reorder request lands in a sheet, a task or your purchasing tool the moment it is filed. Nothing reads stock levels, SKUs or till data back out of the POS, so counts are recorded against your system record rather than posted into it.

Can staff run a stocktake from a phone in the stockroom?

A form is just a link, so the counter opens it on whatever phone or tablet is on the shop floor. The apparel pattern is one submission per style, size and colour combination, filed as they work the rail. What it will not do is calculate the variance — there are no calculated fields. Export the responses and reconcile against the POS figure.

Can seasonal applicants send a CV through the application form?

They can. An upload field sits alongside the availability questions, so a CV and a photo of a food handler card arrive with the application and you preview or download either from the response. The per-file ceiling is 2 MB, so ask for a phone photo of the card rather than a desktop scan. Most of the decision still rests on the plain fields — weekend and holiday cover, till experience, whether the certification the role needs is already in hand — so keep those searchable rather than buried in a PDF.

Can I sell gift cards through the form?

Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.

How should I handle a theft witness statement or a staff conduct complaint?

Both name individuals, and the complaint carries an accusation. Use an end-to-end encrypted form so the suspect description, the store and time of the incident and the employee's account are not sitting in plain text. Hosting is available in the EU and the service is GDPR compliant. A tick-box confirms the account is accurate; it is not a signed affidavit.
