Form templates for photographers
Photography is a trade where most of the work is agreed before the shutter opens. These form templates cover what a photographer actually sends and receives: client intake and shoot questionnaires, package enquiries, wedding shot lists, print orders, usage releases and workshop sign-ups.
The details that decide the shoot are rarely the same twice. A newborn session is booked against a due date, not a diary; a commercial brief turns on usage rights and a campaign deadline; a pet portrait needs temperament notes before anyone unpacks a light.
One form can hold all of it. Conditional logic branches on session type, so a wedding enquiry never asks about SKU counts, and every answer lands in one place you can search, filter and export.
138 forms photographers actually build
Each one starts from a template you can open and edit. Grouped by the kind of form it is.

What photographers usually collect
Due dates and gestational weeks, the profession a headshot is for, mobility considerations for elderly or very young subjects, wardrobe and colour palette, and the pet's temperament — the answers that shape posing, props and lighting.
Where the images will run — web, print, advertising — plus exclusivity period, geographic scope and campaign deadline. Architecture and commercial briefs settle licensing terms up front, because the licence, not the shoot day, is what changes the price.
Parent or guardian approval for images of minors at school events, couple consent to publish a wedding in a portfolio or magazine submission, and retroactive permission from a street subject you photographed months earlier.
Session date, gallery code, package and print size, plus the identifier that matches the frame to the person: bib number at a race, jersey number in a youth league, class number at an equestrian show.

How to set one up
- 1
Open the Photography Client Intake Form template and cut it down to the session types you actually shoot — newborn, headshot, commercial, real estate — rather than keeping every field.
- 2
Add a first question for shoot type, then use conditional logic so a maternity booking asks gestational week while a commercial brief asks SKU count, usage rights and deadline.
- 3
Keep releases separate. Build a second form from Photo & Video Consent Form for portfolio and publication permission, and make it end-to-end encrypted when it names a minor.
- 4
Publish it three ways: embedded on your booking page, as a link in the enquiry reply you already send, and as a QR code for picture day or a race finish line.
- 5
Send each submission onward with a webhook or Zapier so a new enquiry creates a row in your shoot tracker, then filter and export responses by date when the season peaks.
Frequently asked questions
Can clients send me reference images or contest entries through the form?
Yes. File Upload is a field in the editor, so a client attaches the reference shots saved on their phone and a contest entrant attaches the photograph itself. You download or preview each file from the submission, beside the category, caption and entry details. Files are capped at 2 MB each, so a full-resolution frame is better sent as a link — keep a short text field next to the upload for the gallery or drive folder.
Can I take payment for print orders inside the form?
Yes. Add products and priced options to the form, then collect payment through secure hosted checkout with Stripe, PayPal, or Square. Payment status is tracked alongside the response.
Do I need a different form for every type of shoot?
No. One intake form with conditional logic covers them all: choose the session type and the rest of the questions change. A newborn enquiry asks due date and sibling involvement; a real estate booking asks property address, room count and listing deadline. Nobody sees fields that do not apply to them.
Is a model release collected on a form legally binding?
A form can record typed agreement and a ticked consent box, with the usage scope written out beside it. It is not an e-signature product, and there are no signature fields. For school events, parental releases or anything involving a minor, have your own wording checked and keep the form encrypted.
How do parents order photos on the day without a laptop?
Share the print order form as a QR code and put it on the picture day handout, the recital programme or the race finish board. It opens on a phone, and every submission carries the identifier you need — grade and teacher, dancer's class, bib number — so you can filter and export them by class or team later.
