Form templates for hr teams

Form templates for HR teams, covering the whole employment cycle: setup requests before a first day, 90-day check-ins, engagement and pulse surveys, expense claims, and termination checklists. Each opens in the editor as a working form you can rename, rewrite and send.

HR work is calendar-shaped. The annual engagement survey, the benefits poll before open enrolment, the intake of seasonal staff and interns — each comes round again, usually rebuilt from last year's stale copy.

The rest arrives unannounced: a harassment witness statement, a data subject request from someone who left two years ago, a hardship fund application. Those need confidentiality and a clean record, which is what end-to-end encryption is for.

What hr teams usually collect

The joiner handoff

One new-starter record feeds three forms: the IT setup request, the LMS onboarding track, and the 90-day check-in. Start date, reporting manager, role and location — and for interns or contractors, the date access should expire.

Sentiment, without names

Engagement surveys, return-to-office polls and manager effectiveness ratings run on scales plus one free-text box. Department is worth capturing; names usually are not, because the moment a layoff-aftermath poll feels identifiable the answers stop being true.

Expense claim lines

Reimbursement requests need date, amount, category and a policy acknowledgement, plus whatever makes the claim auditable: a CME licence number, or the guest list on a client dinner. A File Upload field carries the receipt itself, so the evidence sits on the same record as the figures.

Offboarding obligations

A termination checklist is a form with a line per obligation: access revoked, laptop shipping label sent, final expenses cleared, benefits continuation explained, reference policy stated. Layoffs, retirements and a death in service each need their own version.

How to set one up

  1. 1

Open the Employee Satisfaction Survey template and cut it back to the questions you will act on. A forty-question annual survey nobody reports back on is worse than twelve with a follow-up.

  2. 2

Add conditional logic so the form branches on employment status — current employee, leaver, applicant — the way a GDPR data subject request has to reach different handling queues.

  3. 3

Decide how it reaches people. A QR code by the entrance suits a daily health screening; the engagement survey wants a link in email; a desk booking form belongs embedded in the intranet.

  4. 4

Switch on end-to-end encryption for anything a complaint, a witness statement or a hardship fund application will carry, and agree beforehand who on the team may read the responses.

  5. 5

Send each submission onward by webhook or Zapier, so a new-starter setup request opens a provisioning ticket the same day rather than sitting in a shared inbox.

Frequently asked questions

Can we run an anonymous employee engagement survey?

Yes. Leave name and email off the form and it collects no identifier. Keep department or tenure if you need to cut the results, but be careful: on a team of six, department plus tenure identifies people, and staff work that out faster than HR does. Say in the intro exactly what is stored.

Can employees attach receipts to a reimbursement form?

Yes. File Upload is a field in the editor, so the photo of a taxi receipt or a conference invoice arrives on the claim beside the amount, date and cost code, and you download or preview it from the submission. Each file is capped at 2 MB, which a phone photo clears easily; a long hotel folio is better linked from a short text field.

Is it safe to take harassment complaints through an online form?

Use an end-to-end encrypted form. The submission is encrypted in the browser, so a complaint or witness statement is readable only by the people holding the key, not by anyone with a general login. EU hosting is available and the service is fully GDPR compliant. It does not replace a documented investigation process.

Does this replace our HRIS?

No. Forms sit in front of it. Workday, BambooHR or whatever you run stays the system of record; the form is how a request or a survey answer reaches it. Every submission fires a real-time webhook, and there is a REST API alongside, so the answers can be written into the HRIS or a ticket queue by whatever you already use, or handed to a Zapier action. The flow runs one way only — a submission triggers something, and nothing is read back out of the HRIS.

Can we collect a signature on a media release or NDA?

A form can record typed agreement: a tick-box and the person's name against the wording shown to them. That is enough for an internal photo consent, or for an NDA enquiry that ends with legal drafting the real document. It is not an e-signature product, so avoid it where an executed signature is required.
