Form templates for law firms & attorneys

Legal intake starts with a stranger describing a problem, and no attorney can return the call until the conflicts check clears. The forms behind that first contact — legal client intake, power of attorney inquiries, callback requests, disbursement invoices — are here as editable formformform templates.

One power of attorney inquiry covers a dozen situations. A military client needs deployment and expiry dates; a snowbird needs both state addresses; an elder law matter asks about the principal's cognitive status. Conditional logic asks only the questions that situation raises.

Firm administration runs from the same editor: conference room bookings that flag the opposing party, coverage forms listing each attorney's filing deadlines, bar association renewals. Because a prospective client's first message is confidential whether or not the firm takes the matter, intake can be end-to-end encrypted and hosted in the EU.

What law firms & attorneys usually collect

The other side's name

Intake is not usable until it records the opposing party, and any related entity, so the conflicts check can run before anyone calls back. Matter type, jurisdiction and how the caller found the firm sit alongside it.

Dates that start a clock

Accident date on a personal injury inquiry, closing date on a real estate matter, deployment and expiry dates on a military power of attorney. Each one tells the firm how much runway a matter has left.

Scope of the instrument

Power of attorney inquiries turn on scope: which financial decisions the agent may take, whether a healthcare directive already exists, whether a special needs trust has to sit alongside it, and how long the authority lasts.

Matter numbers and timekeepers

Vendor invoices from court reporters, expert witnesses and process servers carry a matter number and billing timekeeper so costs reach the right client. Room bookings and desk reservations use the same case reference.

How to set one up

  1. 1

Open the Legal Client Intake Form and rewrite it in the words your intake coordinator uses. Cut the practice areas you never take, and move the opposing party field up for conflicts checking.

  2. 2

Add branches by matter type, so a personal injury caller is asked about insurers and prior claims while an estate planning caller is asked which documents the bundle should contain.

  3. 3

Add the checkbox that records agreement to your no-legal-advice wording, then switch the form to end-to-end encryption before it carries anything a prospective client would not say in public.

  4. 4

Put it on the practice area page with two lines of HTML, add a slide-in on your blog posts about limitation periods, and give reception a link for callers who ring first.

  5. 5

Push submissions out with a real-time webhook or Zapier so a new matter opens in your practice management system. Then build the internal forms — Conference Room Booking, Invoice Request — from the same account.

Frequently asked questions

Can a client upload a contract or accident report with the intake form?

Yes. Add a File Upload field and the client attaches it with the intake — the disputed contract, the police report, a photo of the damage. Files are capped at 2 MB each, so ask for a drive link when a demand package runs long. On an encrypted form the attachment decrypts only in your browser.

Can a client sign a retainer or power of attorney on the form?

No. There are no e-signature fields, and a power of attorney generally needs notarisation or witnesses regardless. What the form does is capture the inquiry — scope, parties, dates — so the attorney can draft correctly and send the execution copy through whatever signing and notary process the jurisdiction requires.

How do we stop a web intake form creating an attorney-client relationship?

That is a wording problem, and the form can carry the wording. Put your disclaimer above the submit button with a required checkbox recording that the caller has read it; the tick is stored with the submission and its timestamp. Confirm the exact language with your own ethics rules.

Is it safe to take confidential client details through a website form?

Put intake on an end-to-end encrypted form and the answers decrypt only in your browser. Everything is encrypted at rest and in transit, EU hosting is available, and the service is fully GDPR compliant. There are no third-party security certifications behind it, so check any outside counsel guidelines that demand one before routing intake here.

Will submissions sync with our case management system?

They push out, not both ways. A real-time webhook or one of 60+ Zapier apps fires the moment someone submits, and your practice management system can open the matter from it. Nothing reads back, so the form cannot check an existing client list or look up a matter number for the caller.
