Form templates for logistics & transport

Form templates for carriers, freight brokers, 3PLs, fleet shops and last-mile delivery operators. Every one opens in a document-style editor as a working draft — an LTL rate request, a Class A CDL driver application, a daily van inspection — ready to reword and publish.

The paperwork follows the load. Dispatch cannot price a quote without freight class, pallet count and both zip codes. A driver application is mostly an endorsement check — hazmat, tanker, passenger, doubles. A dock appointment needs a door number and whoever unloads it.

Peak season multiplies all of it — seasonal warehouse registrations, weekly availability from a gig driver pool, redelivery claims. Conditional logic keeps one form covering reefer, flatbed and hazmat rather than three that drift apart.

What logistics & transport usually collect

CDL class and endorsements

Applicants list class, endorsements and expiry — hazmat, tanker, passenger, doubles — alongside logbook familiarity and the states they have run. Photographs of the licence card itself upload alongside, so the endorsements typed in can be checked against what is printed on it.

Rate request specifics

Pallet count, weight, freight class, pickup and delivery zip codes, ready time and hard deadline. Then the things that move the price: tarps, oversize permits, temperature range, UN number and packing group.

Unit condition and photos

Unit or trailer number, odometer and engine hours, then the system that failed — fifth wheel, air brakes, DEF tank, liftgate, cargo door latch, forklift battery. Photos of the damage attach to the same submission.

Delivery access details

What the driver needs before arriving: dock number and appointment contact, whether the receiver has a forklift, staircase and doorway measurements, gate codes, cooler placement, and whether an adult signature is required.

How to set one up

  1. 1

Open the Freight Shipping Quote Request template and edit it like a document, renaming fields to the words your dispatchers already use — lane, ready time, freight class, accessorials.

  2. 2

Add conditional logic so one form covers every mode: choose reefer and the temperature range appears, choose flatbed and tarping, permits and load dimensions open instead.

  3. 3

Put it where the work happens — two lines of HTML on your rates page, a QR code in the cab for inspection reports, a link in the driver onboarding email.

  4. 4

Send a webhook to your TMS or dispatch board on every submission, and let Zapier push driver applications into recruiting the moment a candidate finishes the form.

  5. 5

Work the responses in one place: filter maintenance requests by unit number, search quotes by destination, export the week's availability before you build Monday's routes.

Frequently asked questions

Can one form handle LTL, flatbed and reefer quote requests?

Yes. One question at the top asks for the mode, and the logic branches from there. Reefer opens temperature range and commodity type; flatbed opens tarping, oversize permits and load dimensions; hazmat opens UN number and packing group. Shippers only ever see the questions that apply to their load, and every quote lands in the same response list.

Can drivers complete a vehicle inspection report from a phone?

Yes. Forms are built to work on a phone screen, so a pre-trip walkaround can be logged from the yard. Tape a QR code inside the cab and the driver scans straight to the form. Photos of a cracked lens or a torn mud flap upload with the submission, so the shop sees the fault before the truck arrives.

How do I collect CDL copies and medical cards from applicants?

Put an upload field under the licence questions and the applicant photographs the CDL, the medical examiner's certificate and any endorsement paperwork from wherever they happen to be parked. Because that packet carries personal identifiers, you can run the application as an end-to-end encrypted form — uploads included — and read it in your own browser.

Can submissions go straight into our TMS or dispatch board?

Every submission can fire a webhook in real time, and there is a REST API if you would rather pull. Zapier covers 60-plus apps for everything else — a rate request creating a card, a maintenance form opening a work order. Traffic only goes outward: the submission triggers the action, and nothing is written back into the form.

What is the best way to collect weekly driver and warehouse availability?

Send the same availability link out every Friday and let each driver or associate tick the days, vehicle type and zip codes they can cover. Peak season is the test: responses are unlimited and free during early access, so a hundred seasonal warehouse registrations in a week cost nothing extra.
