About this template

Patient information update is a separate job from patient intake, and most practices do not have a form for it. The record already exists; one field in it is wrong. Sending a returning patient back through a new-patient intake to correct an address means asking twenty questions to change one answer, and it produces two problems at once — patients who do not bother, and staff who now have a second full submission to reconcile against the first.

This template inverts the required/optional split that a new-patient intake uses. Patient name and date of birth are required because they identify the record, and one confirmation checkbox is required because a record change is acted on. Everything else — new name, phone, email, address, insurer and policy number, GP, referring provider, medications, allergies, emergency contact — is optional. A checklist at the top asks which details have changed so the submission can be routed without reading it all, and an effective-date field handles the common case of telling a practice about a move before it happens.

Two details are deliberate. Medications and allergies ask for the complete current list rather than only what changed, because a list of differences has to be reconciled against an older list by hand and that is exactly where half-updated records come from. Best practices for your patient information update form

Keep everything but the identifiers optional. The entire point is that one change takes one minute. The moment a patient has to re-enter their medical history to correct a phone number, they stop telling you.

Verify the requester before you apply the change. A public form is a request, not an authenticated instruction. Anyone can type a name and a date of birth. Confirm identity through your usual process before changing an address, a name or a funding arrangement.

Ask for the whole current medication and allergy list. A list of changes has to be reconciled by hand against whatever is on file, and that reconciliation is where errors enter a record.

Do not use this form to book anything. It says so at the top for a reason

Attach the message-safety question to the phone number, not to a separate privacy page. It is the moment the answer is relevant, and it is the field people actually change.

Treat a name change as more than a text field. Ask whether it is legal or preferred, and update how the patient is addressed at reception and in letters, not only the top line of the record.

Re-ask sharing consent rather than assuming it carried over. When the referring provider changes, the previous consent was about a different person.

Do not claim compliance. EU hosting and GDPR handling are real and can be stated. SOC 2 and ISO 27001 belong to the hosting provider, and no form makes a practice HIPAA compliant.

How is this different from a patient intake form?

An intake form creates a record for someone you have never seen, so almost everything on it is required. This one changes a record that already exists, so only the two identifiers are required and every information field is optional. Why are only name and date of birth required?

Because they identify the record. Everything else is what the patient came to change, and requiring any of it would force someone correcting a phone number to re-enter details that were already right.

How do we know the person submitting is really the patient?

You do not, from the form alone — and no public form can tell you. Treat every submission as a request and verify the requester through your usual identity process before applying a change, especially for a name, an address or a funding change. Can patients upload a photo of a new insurance card?
