Volunteer forms are not one form but a sequence: someone asks what is available, applies, consents to a check, signs a waiver, gets set up, claims a shift, logs the hours, says what the programme was like, and is eventually nominated for something. This collection gathers ten formformform templates across those nine stages — drawn from the community, legal and government categories, with the application step counted twice because recruiting for a rota and recruiting for a board are not the same job — so you can tell which stage you are actually building for before you clone anything.
Running a volunteer programme is not one form, it is a sequence. Somebody hears about the programme and asks what is available; they apply; you screen them; they sign something; you set up their record; they take a shift; they report what they did; they tell you how the whole thing felt; and eventually a colleague writes down that they were good at it. Each of those stages collects different facts, sometimes from a different person, so this collection follows the sequence rather than listing every template with the word volunteer in its name: ten templates across those nine stages, with the application step counted twice because a rota sign-up and a civic application ask almost nothing in common. It is also why the members are not all filed under community forms: a liability waiver lives in legal forms because it is a waiver first and a volunteer document second, and the civic application lives in government forms because a seat on a board or commission is recruited the way a public appointment is, not the way a Saturday shift is.
The axis that really separates these templates is how each one asks about time. The programme inquiry form asks for Estimated Hours Available Per Month, because at that point nobody has committed to anything. The registration form narrows that to Available Days, Hours Available Per Week and an Earliest Available Start Date, which is enough to build a rota from. The shift sign-up form drops availability altogether and offers Shift Date and Shift Time as dropdowns, because by then the slots exist and you are asking someone to take one. The hours log reverses the direction entirely: Hours Served against a Date of Service that has already happened, naming the supervisor or staff contact who can verify it. The screening form asks about availability not at all — Date of Birth and a criminal-history disclosure in its place — and the waiver asks about time only to date the commitment, with a required Volunteer Start Date sitting beside the risk acknowledgment and no availability question anywhere. Pick the wrong one and you will be asking a stranger for their emergency contact, or asking a scheduled volunteer which days they are free.
Several of these templates contain a question that only makes sense once you have read the answer above it — "If Yes, Please Explain (Nature of Offense, Date, and Outcome)" on the background check, "How Many Additional Volunteers Will You Bring?" on the shift sign-up, "If yes, please describe the conflict of interest" on the civic application, "Miles Traveled (if applicable)" on the hours log. As generated, every one of those ships flat and always visible: formformform's template generator writes no branching, even though the product's own editor supports conditional logic. Clone the template and add the rule in the editor so the follow-up appears only after the answer that calls for it. It is a couple of minutes per pair, and it matters more here than on most forms — a volunteer sign-up that shows a criminal-history explanation box to everyone who opens it reads as an accusation.

How to build a volunteer form
Decide which stage you are building for before you pick a template: recruiting a stranger, screening someone you have already accepted, filling a published rota, or collecting hours after the fact. One form that tries to do all four is why volunteer sign-ups get abandoned halfway.
Ask for availability in the unit you actually schedule in — hours per month while someone is still deciding, days of the week plus a start date for a standing rota, or a shift dropdown when the slots already exist. Asking for all three tells you nothing extra and doubles the form.
Keep screening on its own link. Date of birth, home address, the last four digits of a social security number and a criminal-history disclosure belong on a form you send to people you have already spoken to, never on the public sign-up — and switch that form to end-to-end encryption before you publish it.
Make the emergency contact required wherever the work is physical — the registration form is the only template here that does, while the waiver leaves Emergency Contact Name & Phone optional behind a Yes/No — and keep the risk acknowledgement and the liability release as two separate checkboxes rather than one bundled "I agree", the way the waiver template does.
Add the branching in the editor after cloning, so the conviction explanation, the extra-volunteer count and the conflict-of-interest description each appear only after the Yes that calls for them.
Route submissions to wherever the coordinator already works — a webhook, or a spreadsheet automation that appends each new sign-up to the rota — because between shifts nobody opens a separate dashboard.

Frequently asked questions
Do I need separate forms for signing people up and for booking shifts?
Almost always yes, and the field lists show why. A recruitment form asks Available Days and Hours Available Per Week because you do not yet know what you will schedule; a shift form asks Shift Date and Shift Time as dropdowns you fill with slots that already exist. Merging them means either publishing a rota to strangers or asking a rostered volunteer to re-declare their availability every time they take a Saturday. Use the registration form once per volunteer and the shift sign-up form once per shift.
Can volunteers upload a certificate, an ID, or a signed waiver?
Not in the templates as generated, but yes in the product. formformform's editor has a File Upload field in its palette — 2 MB per file, encrypted on encrypted forms — it is simply not one of the field types the template generator can write, so nothing you clone from this page arrives with one. Add it in the editor after cloning. There is no e-signature field at all, which is why the waiver template takes a typed Volunteer Full Name, a Volunteer Start Date and two required acknowledgement checkboxes instead of a drawn signature; if you need a countersigned document, send it through your e-signature tool and use the form for the intake around it.
Is it safe to collect background-check answers through a form?
It is the most sensitive form in this set, so treat it accordingly. formformform is GDPR compliant and hosted in the EU, and any form can be switched to optional end-to-end encryption, which is worth turning on before you ask for a date of birth, a home address and a criminal-history disclosure on the same page. Be precise about what that does and does not give you: SOC 2 and ISO 27001 certifications belong to the hosting provider, not to formformform, and nothing here makes your organisation compliant with the background-check rules of your own jurisdiction. Ask for the check consent separately from the sign-up, keep the responses to the people who decide on placements, and delete them when the decision is made.
Do the rating questions on the volunteer feedback form come as a star or slider scale?
No — formformform has no rating or scale field type at all, so every rated question ships as a radio group with labelled options. On the feedback template the four questions in its Program Ratings block carry four options each, and they are not all the same four: Overall Volunteer Experience, Organization and Logistics and Staff and Coordinator Support each run Excellent, Good, Fair, Poor, while Sense of Impact and Purpose runs Strongly felt, Somewhat felt, Unsure, Did not feel impact. The question that closes the block, Would You Volunteer With Us Again?, runs five — Definitely yes through Definitely not — so the form does not average on a single scale as it stands. That exports cleanly to CSV and is fine for trend data, but it renders as a list of radio buttons rather than a row of stars, so do not promise a star rating in the email that links to it — and if you want every rated question to average on one scale, edit the options to match in the editor before you send it.
How do I collect volunteer hours without building a timesheet system?
The hours submission form is a per-session log, not a clock: one entry per shift, with Hours Served, a Date of Service, the Program or Project Name, a Supervisor or Staff Contact Name to verify it, and Miles Traveled for anyone reimbursing mileage. Send the same link after every shift and total the responses in your spreadsheet or reporting tool. The one change worth making first is the Program or Project Name: it ships as a short text field, so two volunteers on the same programme will spell it two different ways — switch it to a dropdown of your actual programmes in the editor after cloning, and that is the difference between a report you can total and a column you have to clean by hand. To match hours back to a person rather than a programme, group on the Email Address rather than the typed Volunteer Name.
