About this template

Food traders are the hardest vendors an event has to plan for, because a food unit is not a stall — it is a kitchen that arrives on wheels and has to be connected to things. It needs a footprint that includes the towbar and the open hatch, a height that clears the archway, a power supply measured in amps at peak rather than in vague terms, potable water for handwashing, somewhere legal for grey water and waste cooking oil to go, and gas cylinders stored to a standard someone will check. Miss any one of those at application time and it surfaces on setup morning, when there is nothing to be done about it.

The compliance half of this form is written to be checkable rather than reassuring. Most vendor forms ask whether a trader has permits and insurance and accept a yes. This one asks for a food hygiene rating and the authority that issued it, so the rating can be looked up; for a gas safety certificate expiry date, so a certificate that runs out between June and September is caught in June; and for public liability and product liability as two separate answers, because for someone serving food they are genuinely different risks — a customer tripping over a cable, and a customer becoming ill from what they ate. Allergen handling is an open question on purpose. A checklist of the fourteen allergens tells an organiser what is on the menu; asking how cross-contamination is prevented and who is trained tells them whether there is a system behind the menu.

Best practices for your food vendor application form

Publish your power provision in amps and connector types before applications open. "Power available" means nothing to someone deciding whether to bring a 7 kVA generator.

Ask for peak draw, not average. Fryer, griddle, extraction, lighting and a fridge all running during a lunch rush is the number that matters, and the one that trips a row when it is understated.

Set and publish a minimum food hygiene rating. Traders will meet a stated bar; they cannot meet an unstated one, and rejecting on an unpublished standard is how organisers acquire reputations.

Check the rating against the registering authority rather than believing the form. That is the whole reason this template asks for the authority's name alongside the number.

Treat the gas certificate expiry date as a diary entry, not a field. Anything expiring before the event needs chasing the week you receive it, not the week of the event.

Arrange the grey water and waste oil route before you accept traders who need one. It is not a detail you can improvise on a field at eleven at night.

Read the allergen answer properly, and follow up on hedging. A trader who says plainly that they cannot serve a severe nut allergy safely is being more useful than one who says all allergens are catered for.

Ask for the towbar and the open hatch in the footprint, and plan pitches from the largest dimension. Units are measured closed and traded open, and the difference is usually a metre.

Confirm acceptances in writing with the pitch, the sessions, the fee and the setup window, and treat that message as the booking. This form is the application.

Collect only the compliance evidence you will check, and delete responses on the schedule you tell traders about

insurance, certificates and contact details are not data to keep indefinitely because the form made it easy.

Frequently asked questions

Can this form collect the pitch fee or a deposit?

No. formformform has no payment field of any kind — the only payment processing in the product is subscription billing for your own account, and nothing a respondent fills in can charge them. This form records which sessions a trader is applying for and what unit they are bringing, so that when you offer a pitch you already know what to invoice for. Take the fee through whatever you use now: an invoice on acceptance, a bank transfer, or a card payment link in your confirmation email. That separation suits food events particularly well, because pitch terms are often negotiated or set as a percentage of takings after acceptance.

How is this different from the general Event Vendor Registration Form?

The general vendor form asks every trader the same questions and covers permits and insurance with two yes-or-no fields. A food unit needs answers that form has no room for: footprint with height and towbar, peak amperage and connector, generator noise rating, potable water, grey water and waste cooking oil routes, gas safety certificate expiry, food hygiene rating and the authority that issued it, allergen handling, and product liability as distinct from public liability. Use the general form for a mixed market; use this one when the applicant is cooking.

Is this the same as the Food Truck Booking Inquiry Form?

No — it runs in the opposite direction. The booking inquiry form is filled in by an event organiser or host who wants to hire a food truck, and it asks about their event: date, venue, guest count, hours of service. This form is filled in by the trader applying to an event that already exists, and it asks about their unit and their compliance. Same two parties, opposite respondent.

Can traders attach their hygiene certificate, insurance documents or a risk assessment?

Not as the template ships, because the template generator cannot create upload fields — but formformform supports them. The template asks for dates and reference details by default because a date you can sort and chase is more useful to an organiser than a PDF nobody opens.

Why are public liability and product liability asked separately?

Because for a food trader they cover different things. Public liability covers injury or damage to third parties — a customer tripping over your cable, or a gazebo landing on someone. Product liability covers harm caused by what you sold, which for a food business is the risk that actually keeps organisers awake. Many trader policies bundle both, which is why "included within my public liability policy" is one of the answers rather than something the form quietly assumes.

Does this form make my event or my traders compliant?

No, and no form can. It collects the evidence you need in order to check compliance yourself and to have a record of what each trader told you. Registration, inspection, gas certification and insurance are all things the trader holds and the relevant authority verifies. Set your standards in your trader terms, check what you are told against the issuing authority, and keep the responses as your audit trail.

Can I use this for indoor markets and food halls as well as festivals?

Yes, with edits. Indoor sites usually rule out gas and generators entirely and provide fixed power, so delete or reword those fields and put your provision in the terms. Keep the hygiene rating, registering authority, allergen and insurance questions unchanged — those apply wherever the trader is standing.

How many applications can I take?

As many as arrive. formformform does not price by response, so a festival with four hundred trader applications costs the same as a market with ten.
