An open house registration form is a digital replacement for the paper sign-in sheet that visitors used to scribble illegibly at the door. The paper version produced wasted leads — half the email addresses were unreadable, half the phone numbers were missing area codes, and the agent or school admissions officer spent an hour after the event trying to reconstruct who came and what they wanted.
This template is intentionally short. Open house visitors are in a hurry, often standing in a hallway or at a kitchen island, and every extra field reduces completion rates. It captures the basics — name, email, phone, location — plus three short questions: how they heard about you, whether they're working with someone already, and what they'd like answered. That's enough information to follow up intelligently without making anyone feel like they're filling out a tax return.
The form works for any kind of open house: real estate listings, private school admissions days, university campus tours, daycare facility tours, gym walk-throughs, co-working space tours, and grand openings. Embed it on your event page, generate a QR code that visitors can scan with their phone, or load it on a tablet at the door for in-person sign-in. formformform charges nothing per submission and never caps your visitor count, so even a high-traffic Saturday open house collects clean data without hitting any limits.

8 fields or fewer. Open house visitors are mid-stride and won't fill out anything that feels like work.
many visitors prefer to use their own phone, and a QR code is faster than a sticky tablet keyboard.
this single field tells you which marketing channels actually drive foot traffic, which is the most useful piece of marketing data you can collect.
visitors are wary of giving out a phone number to a stranger. Email is enough for follow-up.
open house leads cool fast. A note that references something specific from their visit ("Thanks for stopping by — I noticed you spent some time in the kitchen, here's the appliance package list") converts dramatically better than a generic thank-you.
it gives visitors space to share what's actually on their mind without feeling interrogated.
