Loan and credit inquiry form templates
A loan inquiry form collects what a lender needs before anyone underwrites anything: how much, what for, what the borrower earns, and roughly where their credit sits. This collection gathers the formformform templates built for that pre-application step — mortgage, refinance, home equity, personal, business and open trade credit — and not one of them makes a decision, runs a check, or approves anything.
A loan inquiry form is the step before an application. Somebody says how much they want, what it is for, roughly what they earn and roughly where their credit sits, and a human being decides whether there is a conversation worth having. Everything on this page belongs to that one step, the Credit Application Form included — it is named for the account it opens rather than for the paperwork it takes, and it asks for no tax ID, no references and nothing to sign. That is also why every member sits in finance-forms rather than spreading across the library the way most collections here do: lending questions only look like intake questions. Three finance templates that read as if they belonged were left out — Banking Service Inquiry Form offers checking, savings, CDs, money market accounts and safe deposit boxes and never mentions borrowing; Credit Union Membership Inquiry Form is an eligibility check with lending as a follow-up flag; Financial Hardship Application Form is about credit somebody already has.
What separates these templates is what the loan is secured on and what the lender already knows. Three of them ask for a property address. The refinance form asks for the current loan balance, the current interest rate and the current monthly payment, and asks nothing at all about income or employment, because the loan being replaced already describes the exposure. The home equity form does the opposite, taking Estimated Home Value and Current Mortgage Balance as two separate figures so the equity is arithmetic rather than a claim. The mortgage form asks for the address only if the enquiry is a refinance — the label says as much — because a buyer may not have picked a house yet; what it insists on instead is a Purchase Price or Estimated Home Value. Where a property is not involved, the questions turn to the borrower — an Employment Status list on the personal loan and credit application forms, an annual income figure on both, and, on the personal and business forms, a collateral question that decides whether the loan is secured at all. Five of the six ask for credit as a self-reported band, always the same brackets from "760 or above" down to "Below 600", and always with a last option for a borrower who has never checked, worded "I don't know" on the refinance, home equity and business forms and "I don't know my credit score" on the mortgage and personal loan ones. The sixth, the Credit Application Form, asks for no credit band whatsoever.
The constraint that bites hardest here is documents. Real lending runs on pay stubs, tax returns, bank statements and entity paperwork, and none of these templates arrives with a place to put them: formformform's editor has an Upload field, but its template generator cannot seed one, so every template on this page ships without it. Add the field yourself after you clone — it is in the Upload section of the editor palette, it accepts 2 MB per file, and on a form with end-to-end encryption switched on the uploads are encrypted too. If the paperwork you need is heavier than that, ask for it at the application stage instead and keep the enquiry form to a Long Text field where someone can describe what they can supply.

- Mortgage Inquiry Form
Start here when the enquiry is about buying, because this is the only member that asks for a Down Payment Amount alongside the purchase price, and the only one that lets someone name a loan programme — Conventional, FHA, VA, USDA or Jumbo — and a mortgage term, 15, 20 or 30 years fixed or an adjustable rate. Both lists end in a not-sure answer, "Not sure — help me decide" on the programme and "Not sure" on the term, so an undecided buyer can still submit.

Use it when the borrower already has the loan you would be replacing — it is the only member with an interest-rate field at all, and the only one that asks what the existing loan costs each month. It asks the outstanding balance too, though the home equity form asks for that as well; the rate and the payment are what let you check the arithmetic behind its Primary Refinance Goal answer, one of seven that include "Lower my interest rate", "Cash-out equity" and "Remove PMI".

Use this for an account with a limit rather than a single advance — trade credit, a supplier line, equipment or inventory financing. It is the only member with no credit-score question at all, judging stability instead by address history (Years at Current Address, own or rent), and the only one where the applicant's own annual income is a required answer rather than an optional one. It is called an application, but it stops where the rest of the page stops: it asks for no tax ID, no trade or bank references and nothing to sign, so opening the account is still work somebody does afterwards.

How to build a loan and credit inquiry form
Decide what the loan is secured on before you write a single field — a property, a business asset, or nothing — because that one choice determines whether you ask for a home value, an annual revenue, or a plain collateral yes-or-no.
Put the amount and the purpose at the top and make the purpose a fixed list rather than free text, so debt consolidation and equipment purchase arrive as the same string every time and route themselves.
Ask for credit as a self-reported band with an explicit not-sure option, never as a number, and say in the label or the form's intro that nothing here runs a credit check or pulls a bureau file.
State plainly on the form that submitting it is an enquiry, not an application, and that no offer, rate or approval follows from it — this is a compliance sentence, not marketing copy, and it belongs above the submit button.
Leave national ID numbers off the enquiry entirely. Collect an income figure and an employment status if you must qualify, turn on end-to-end encryption for the form, and gather identity later in the process where it belongs.
After cloning, add an Upload field in the editor for proof of income, and add logic so the fields whose labels currently read "(if refinancing)" or "(if applicable)" appear only when they apply — then webhook submissions to whoever works the pipeline so an enquiry is not sitting in a dashboard nobody opened.

Frequently asked questions
Do these forms approve anyone or check their credit?
No, and none of them can. Every template here collects what somebody says about themselves — an amount, a purpose, an income figure they typed, a credit band they picked from a list. Nothing in formformform pulls a bureau file, scores an applicant, or returns a decision, so a submission is an enquiry that a person then reads. If your form implies otherwise, that is a regulatory problem rather than a copy problem: say on the form that it is not an application and that no offer follows from it.
How should I ask for a credit score?
As a band, from a fixed list, with an escape hatch. Five of these six templates do exactly that, using the same brackets — 760 or above, 720–759, 680–719, 640–679, 600–639, Below 600 — plus a final not-sure option, written "I don't know" on three of them and "I don't know my credit score" on the mortgage and personal loan forms. That last option matters more than it looks: a borrower who has never checked their score can still submit rather than guessing a number that then reads as a fact. The exception, the Credit Application Form, skips the question altogether and judges stability from address history and required income instead.
Can applicants attach pay stubs, tax returns or bank statements?
Not on the template as generated, but yes on the form once you have cloned it. formformform's editor has an Upload field — it is in the Upload section of the palette, it takes 2 MB per file, and it is encrypted on encrypted forms — the template generator simply cannot seed one, so no template on this site arrives with an upload field already placed. Add it yourself. For anything larger than 2 MB, keep the enquiry form light and collect the paperwork at the application stage.
These forms collect income and identity. Where does that data actually go?
To EU-hosted infrastructure, under GDPR, and you can switch on optional end-to-end encryption per form so responses are unreadable outside your own session. What formformform does not have is SOC 2, ISO 27001 or HIPAA attestation — if your lender or compliance team requires one of those, this is not the right place for the data. The honest advice beyond hosting is to collect less: none of the templates on this page asks for a national ID or social security number, and an enquiry form has no business asking for one either.
Should I run one credit form or a separate form per product?
Separate, in almost every case, because the questions barely overlap. A refinance enquiry needs a current rate and balance that a personal loan enquiry has no use for; a business enquiry needs an entity type and revenue that a mortgage enquiry does not ask about. A borrower answering fields that do not apply to them abandons the form or answers them badly.
