Event RSVP Form Template
An RSVP is a reply to an invitation, not an open sign-up. This template is built for a fixed guest list and any kind of event: the first required question is whether you're coming, declines are a first-class answer rather than an abandoned form, and the plus-one field asks whether you're bringing the guest you were invited with instead of letting anyone add seats.

About this template
An RSVP form and an event registration form look similar and do opposite jobs. Registration opens a list: anyone with the link puts themselves on it, and the organiser's problem is capacity, ticket types and follow-up. An RSVP closes a list that already exists: you know who was invited, you know how many seats you offered each of them, and the thing you're missing is a yes or a no. That is why this template's first required question is whether the guest is coming, and why the name field asks for the name as it appears on the invitation — you're ticking people off a list, not adding them to one.
formformform already had two RSVP templates before this one, and both were locked to an occasion. The Party RSVP Form is six questions built for a headcount: attending, how many, dietary restrictions, anything else. The Wedding RSVP Form asks for a meal preference and a song request and a message for the couple. Neither works for a client dinner, a donor evening, an awards night or a company anniversary, and the Event Registration Form doesn't either — it asks for ticket type, organisation and emergency contact details, which are open-signup questions no invited guest expects to answer. This template sits in the gap: the same reply-to-an-invitation shape, with the occasion-specific fields replaced by ones you rename to your own event.
A few things this form deliberately doesn't do. It doesn't take money — formformform has no payment field, so if your event is ticketed, this is the wrong template and the wrong product for the transaction. It doesn't ship with conditional logic, because the template generator writes every field flat; if you want the meal choice to appear only after someone answers yes, that's a short change in the editor after you clone it. And it can't enforce your guest list on its own: a form link works for whoever holds it, so if your list matters, share it person by person rather than posting it publicly, and treat a name you don't recognise as a question rather than a booking.

How to create an event RSVP form
- 1
Click "Use this template" to load the RSVP form into your free formformform account.
- 2
Put the event name, date, time and venue into the heading and the opening paragraph, along with the date you need replies by — an RSVP form with no deadline gets replies after you've confirmed numbers with the caterer.
- 3
Rename the meal options to your actual menu, or delete the meal field if there's no catering, and rename the "which parts will you join" options to your run of show.
- 4
Decide your plus-one policy before you publish, and edit the plus-one options to match it — if no invitation allows a guest, delete the field and the seat count together.
- 5
Add conditional logic in the editor if you want the meal and party fields to appear only after someone answers yes; the template ships flat because the generator can't store branching.
- 6
Share the link in the invitation itself — by email, message or a QR code on a printed card — rather than posting it publicly, since the form can't check anyone against your guest list.
- 7
Send a reminder a few days before your deadline to everyone who hasn't replied; "not sure yet" answers are the ones worth chasing personally.
Best practices for your event RSVP form
a guest who is declining should be able to finish in three fields, and a form that demands a meal choice from someone who isn't coming just gets abandoned.
Give people a "not sure yet" option. Without it, the undecided don't answer at all, and you can't tell them apart from the people who never opened the invitation.
the caterer's cut-off is the real deadline and guests can't guess it.
Collect names, not just a number. "Party of 4" is a headcount; four names are a table plan, four badges and four correct meals.
Bound the plus-one question by what you actually invited. "May I bring someone?" is a request you answer; "I'm bringing the guest named on my invitation" is a confirmation. Keeping them as separate answers stops your seat count drifting.
the need someone has is usually not on the list, and an open field gets you the real answer in time to do something about it.
Share the link privately, one invitation at a time. A form link is public to whoever holds it, so if the guest list is the point, don't post the link anywhere it can be forwarded onward.
Treat the latest reply as the current one. People change their plans; ask guests to submit again if something changes, and check the timestamp rather than assuming the first answer still stands.
attending, how many, which meal — so mistakes get corrected before the numbers go to the venue.
Frequently asked questions
What's the difference between an RSVP form and an event registration form?
Who starts the conversation. An RSVP replies to an invitation you already sent to a known guest list, so the questions are about that invitation: are you coming, are you bringing the guest we offered you, what will you eat. Registration opens a list to anyone with the link, so it asks for ticket type, organisation and the details you need from a stranger. If you know everyone's name before the form goes out, you want an RSVP form. If you don't, you want formformform's Event Registration Form instead.
Can I collect ticket payment or a deposit with this form?
No. formformform has no payment field — the only payment processing in the product is subscription billing for your own account, not anything a respondent can pay into. This template is for invitations where the seat is already yours to give. If money has to change hands, collect it through whatever you already use to take payments and use the RSVP purely for numbers and dietary details.
How do I stop guests inviting extra people?
The plus-one field is written to do exactly that: bringing the guest named on the invitation and asking whether you may bring someone are two different answers, so an unapproved guest arrives as a request rather than a claimed seat. If your invitations are strictly one seat each, delete both the plus-one field and the seat count so the question never comes up.
Can the form check people against my guest list automatically?
No, and it's worth being clear about that before you publish. A form link works for anyone who has it, and this template has no way to look a name up against a list. Send the link inside the invitation itself rather than posting it publicly, and treat a reply from a name you don't recognise as something to follow up rather than a confirmed seat.
Does the meal choice field appear only for people who are attending?
Not as the template ships. formformform's template generator writes every field flat, so nothing here arrives with branching even though the editor supports it. Clone the template and add the logic yourself — show the meal, seats and party fields only when the attendance answer is yes — and it's a few minutes' work, not a missing feature.
Can I use one form for several events?
You can, but replies get harder to sort. The cleaner approach is to duplicate the form for each event so the heading, deadline, menu and run of show are all correct on every copy, and each guest list gets its own link and its own set of responses.
What should I do about declines?
Count them. A decline is what tells you a seat is free, and it's the only way to tell someone who can't come apart from someone who never opened the invitation. That's why declining takes three fields on this form and why the "anything else for the host" box invites a reason without demanding one.
Can guests change their answer after they've replied?
Ask them to submit the form again — you'll get a second response with a later timestamp, and you treat the most recent one as current. If plans change often for your event, say so in the confirmation message so guests know resubmitting is the right thing to do rather than emailing you separately.
