About this template

formformform's Job Application Form is a good general hiring intake: position applied for, employment type, available start date, highest level of education, work experience, skills, cover letter. Point a student at it and roughly half of it misfires. Employment type has no answer. Work experience asks for a history the applicant is applying in order to start building. And the fields that decide whether a placement can go ahead at all — the credit requirement, the fixed term window, the academic supervisor — have nowhere to go.

This form is built around the academic calendar instead. It asks for the institution, the programme, the year of study and the expected graduation date; then whether the placement is required for credit, and if so how many hours or weeks the programme demands. It asks for both an earliest start and a latest end, because a placement window is usually fixed by a term rather than negotiated. It asks which days the applicant can be on site, which matters more than a weekly total when teaching is timetabled. And it asks a question no job application asks: what does your institution need from us — a learning agreement, a mid-point review, an insurance certificate, a site visit, a final evaluation.

The centre of the form is the learning goals field, and it is required. A placement that produces no learning is a short unpaid job, and the answer here is what a supervisor rereads when they write the end-of-placement evaluation months later. Around it, evidence is asked in terms a student can actually answer — coursework, projects, societies, part-time work, anything built or run — with optional links to a portfolio, a repository and a CV. Sensitive fields stay minimal: the visa question asks only whether paperwork will need signing, never for passport, visa or identification numbers, which belong in onboarding rather than on an application form.

Best practices for your internship application form

Ask about the credit requirement first, not last

it determines the paperwork, the minimum hours and the evaluation deadline, and everything else you plan follows from it.

Collect both ends of the placement window. A single "available start date" field lets you offer a twelve-week placement to someone whose term restarts in eight.

Make the learning-goals field required and read it before the coursework field. A specific answer here predicts a good placement far better than a long list of modules.

Ask which days are free rather than only how many hours. Timetabled teaching does not move, and Tuesday-and-Thursday availability is a different placement from three days of your choosing.

Never ask for passport, visa or national identification numbers on an application form

ask only whether paperwork will need signing, and collect the rest at onboarding in a system built for it.

Delete the fields that only make sense for employment. Salary expectations, notice period and employment type all read as copied-in from a job application and tell a student you haven't run a placement before.

Reply on a term timetable. Placement applications are often submitted against an institutional deadline, and a three-week silence can cost a student the module.

Say what the placement pays in the posting rather than asking the applicant. Whether an internship is paid is your policy to state, not a negotiation to open on a form.
