The nearest thing in formformform's library is the Staffing Agency Candidate Registration form, and the difference is the shape of the commitment. That form registers a candidate with an agency indefinitely: work authorization, job type sought, industries of interest, desired pay rate, shift availability, transport, and consent to background and drug checks. It is a standing profile, and there is no season in it — no start date, no end date, no peak week, no last year. This form applies to one employer for one window.

So it starts with two dates: the earliest someone can start and the last day they can work. Then it asks whether that covers the season as advertised, and gives space to name the gaps already in the calendar — the exam week, the holiday booked in June. Shift and day availability come next as checkboxes, with a separate question about public holidays and peak dates that you are expected to rewrite with your actual dates in it, because "public holidays" means nothing to an applicant when what you need is the two weeks before Christmas. Best practices for your seasonal worker application form

Ask for both ends of availability. A single start date hides the applicant who leaves three weeks before your season ends, which is the shortfall that costs the most to fill.

Name your peak dates explicitly instead of writing "public holidays"
