Before arrival they qualify: travel and tour enquiries, vacation rental and group room block requests, ski lodge reservations, venue enquiries. During the stay they dispatch: concierge requests, room preferences, maintenance reports, activity bookings. Afterwards they measure and retain: post-trip ratings, loyalty enrolment, preference questionnaires.
Underneath every booking and enquiry form sit two numbers: a date range and a headcount. Check-in and check-out, departure and return, guests, rooms, participants. The in-stay and post-stay forms drop both and key on a room or reservation number instead.
What is a hospitality form?
A hospitality form is an intake tied to a stay rather than a sale. Most are requests: a traveller proposes dates and a party size, and your team replies with availability and a rate. In-stay forms are the exception — a maintenance report is a work order keyed to a room number and an urgency, and it needs a duty manager, not a sales inbox.

Why use travel & hospitality form templates
Group blocks and venue enquiries vary by an order of magnitude. Room count, dates and a budget range say whether this is a fifteen-room weekend or a conference.
An enquiry form on your own site takes the guest before a listing platform does, with no commission deducted. Ask about date flexibility and a full week becomes a counter-offer.
A QR code by the bed takes a broken heater at 11pm without a hold queue, and graded urgency plus a webhook alerts a duty manager rather than a shared front-desk inbox.
How to create your own

For anything pre-arrival, open with the date range and make it required — nothing else on the form can be answered without it.

Ask the headcount next: guests, rooms or participants. It drives capacity, staffing and price.

For enquiries, add the qualifiers you would otherwise spend a call on: budget range, room type mix, meeting space, tour type.

For in-stay forms, require the room or reservation number, then add urgency, entry permission and a preferred time.

Use logic so an add-on tick box reveals a quantity field, then share by in-room QR code or an embed on the booking page.
Getting better responses
A tick box marked "lift tickets" or "airport shuttle" cannot be priced. How many, and for how many days, is the difference between an enquiry and a same-day quote.
Inside two days you get the guide's name, the room number and the thing that went wrong. After that you get a score and no way to act on it.
A dated form reads like a confirmed booking. State in the intro that it is a request, give a response time, and hold to it.

What should a hotel or vacation rental enquiry form ask?
Check-in and check-out dates, number of guests, and the property or room type they have in mind, then contact details and an open field for questions about pets, parking or accessibility. A date-flexibility question is what lets you rescue an enquiry for a week you are already full. Keep it under ten questions — travellers are usually comparing several properties at once.
Is a form submission a confirmed booking?
Not unless you say so. Treat it as a request: the guest proposes dates and you confirm availability, rate and deposit separately. Say this plainly in the form intro and again in your reply, because a traveller who submitted dates and heard nothing will arrive expecting a room.
How do I handle enquiries for dates that are already booked?
A form does not read your live calendar, so build the reply into the process instead. Ask whether the dates are flexible and by how much, then answer an unavailable request with the nearest open window rather than a bare decline. For tours and resort activities, offering a second time slot in the same message recovers a good share of them.
Can guests fill this in from their room or their phone?
Yes. Share the form as a QR code on a tent card, the TV screen or a bathroom mirror card, or embed it in your guest portal with two lines of HTML — the script is under 8kb gzipped, so it loads on hotel wifi. No app install is involved and there is nothing for the guest to sign in to.
