Why use alumni form templates
Graduation year sits on almost every template because it is the join key. With degree and email beside it, an update lands on the right person rather than as a stranger.
"Happy to help" is not a plan. Hours per month, day and time slots, in-person or virtual, and a skills field turn a willing graduate into a name you can put on a panel.
An employment status dropdown, a degree relevance question and one rating scale across instruction, career preparation and network give you a dataset, not a folder of anecdotes.

Getting better responses
Alumni are volunteering information, not applying for something. Require name and email; leave employer, city and LinkedIn optional. A half-updated record beats a stale one.
Vague availability is why mentors and volunteers go quiet after signing up. Hours per month as a number and time slots as checkboxes set the expectation — then honour what they picked.
A nomination is the one form here whose subject is not the person typing. Give each a section, swap "tell us about them" for prompts on merit, accomplishments and impact, and ask whether the nominee knows.

How often should alumni be asked to update their details?
Once a year is the usual rhythm, attached to something already happening — reunion season, a giving day, a class newsletter — so the request arrives with a reason rather than on its own. Keep the same form live in between for people who move house or change job, and link to it from your alumni page so it stays findable.
When should an alumni outcome survey go out?
Six months and one year after graduation are the common windows: long enough for graduates to have found work, recent enough that they still remember the teaching clearly. Decide upfront whether responses are anonymous, because anonymity tends to produce franker criticism but removes your ability to match answers back to a graduate record. If a reporting body sets your survey requirements, confirm those against their own guidance.
Should award nominations be open to anyone, or invitation only?
Open calls surface candidates that staff recommendations miss, which is usually the point of running the programme. If you open it publicly, decide in advance whether self-nomination is allowed and say so on the form. Include a question asking whether the nominee is aware — a surprise nominee who declines leaves the committee stranded.
How do submissions reach our alumni CRM?
Every submission fires a real-time webhook, and there is a REST API plus 60-plus app integrations through the published Zapier app. The flow is one-directional: a submission triggers an action in the other system, so it can create or update a contact there, but it never reads data back. Responses also stay in formformform, where you can filter, search and export them.
