formformform's legal forms cover the paperwork a small organisation has to keep on file: waivers, NDA briefs, data-subject requests, witness statements. Consent and release is one task inside that range, and it is really two closely related jobs. Consent records that a person agreed to something being done to or about them — a remote appointment, a study, a background check. A release grants permission to use something that already exists — a photograph, a recording, a quotation, a likeness. Permission is the third case, where somebody authorises on behalf of a person or animal who cannot answer for themselves. That split is why this collection reaches outside legal-forms: a telehealth consent sits with healthcare forms, a treatment authorisation with veterinary forms, a usage request with photography forms, a background-check authorisation with community forms and a testimonial permission with the survey templates, and every one of them is doing one of those three jobs.
The axis that actually separates them is who is answering and how tightly the permission is scoped. Photo & Video Consent asks "Is this consent for a minor?" and offers "No — I am the subject and I am 18 or older" or "Yes — I am the parent or guardian of the minor subject"; the Field Trip Permission Form carries no such branch because a parent is the only person it ever expects; the Veterinary Treatment Consent Form settles it in its acknowledgment, "I confirm I am the legal owner or authorized agent for this animal". Scope shows up in the option lists just as plainly. Media & Publicity Release offers "Perpetual — I grant this release with no expiration date" against a time-limited release with an expiry date. Photo & Video Consent offers a consent revocable in writing against one that is irrevocable. The veterinary form scopes authority in currency, at $500 without further contact. And where a blanket yes would be too crude, the permission is broken up: Field Trip Permission asks about attendance and photography separately, the volunteer authorisation ticks a criminal record check and a reference check as two boxes, and the testimonial form grades attribution three ways.
The constraint that bites hardest on this particular page is signatures. formformform has no e-signature field, and none of the nine templates above contains one — the Media & Publicity Release even labels its button "Sign & Submit Release", and that label is as close as any of them gets. What actually lands in your responses is a typed full name — all nine ask for one and require it — the answers the person selected, and a timestamped submission. Six of the nine take the agreement itself as a required checkbox. The Field Trip Permission Form and the Customer Testimonial Submission contain no checkbox anywhere and put every permission in a radio instead, "Yes, I give permission for my child to attend this field trip" and "Yes, use my full name"; the Photo Usage Permission Request has none either, because nobody grants anything on it — it is the photographer's enquiry, and the subject's answer comes afterwards. A ticked box and a chosen radio are worth exactly the same here: a dated, searchable record that a named person agreed to stated terms, which is what most organisations keeping consent on file actually need. It is not an executed document, and it should never be described as one. If you need more than a record, the editor is where you fix it — add a typed-name confirmation and a date field, add a File Upload field for a countersigned page or a guardian's ID (the templates cannot be generated with one, but the editor palette has it, at 2 MB per file and encrypted on encrypted forms), and pass the submission to a signing service if the document has to be executed rather than filed.

How to build a consent and release form
Decide first whether you are recording consent to an act or a release to use something, then write what is being agreed to into the form itself — a Long Text field the way the Research Study Consent Form carries Study Purpose and Procedures, so the stored response shows what the person actually read.
Name who is answering before you write the acknowledgment: add an "am I the subject or the guardian?" radio if either can apply, a guardian-name field that follows it, and an explicit confirmation of authority when someone is answering for an animal, a child or an estate.
Break the permission into the pieces a reasonable person might refuse separately — one checkbox option per channel, one radio per distinct permission — rather than a single blanket yes, so a no to social media does not cost you the whole consent.
Put an end on it: a perpetual-or-expiring choice plus an expiry date for a release, or a stated revocation route for a consent, and remember that the statement is a promise your process has to keep, not something the form can enforce.
Make the acknowledgment a required answer of its own — a checkbox the way six of these nine do it, or a radio the way the Field Trip Permission Form does — next to the person's typed full name, label the submit button for what it does, and turn on end-to-end encryption in the editor before the form goes live if it will hold dates of birth, health notes, criminal-history answers or the last four digits of a social security number.
Route the submission where the record has to live — a webhook into your case system, or a signing service such as DocuSign if the agreement genuinely has to be executed — and publish the address that revocations go to, because a withdrawal has to reach a person, not a form.

What is the difference between a consent form and a release form?
Consent is agreement to something being done to or about you — treated over video, studied, recorded, background-checked. A release is permission to use something that already exists or is about to: your image, your likeness, your words. The distinction shows up in the fields. The Telehealth Appointment Consent Form asks you to consent to receiving care remotely; the Media & Publicity Release asks you to authorise formats, grant editorial rights and set a duration, because the organisation is going to do something with the content afterwards. A third case, often called a permission slip, is neither: the Field Trip Permission Form and the Veterinary Treatment Consent Form are both completed by someone authorising on behalf of a person or animal who cannot answer, which is why each one identifies the answering party in its own way — the field trip form gives the parent or guardian a whole section of name, email, phone and emergency contact, and the veterinary form makes the owner tick a confirmation that they are the legal owner or authorised agent.
Can someone sign a consent or release form electronically in formformform?
Not as a signature, and it is worth being precise about it. formformform has no e-signature field, so none of these templates contains one. What you get is a typed full name — all nine ask for one and require it — the answers the person selected, and the submission timestamp. On six of them the agreement is a required checkbox; the Field Trip Permission Form, the Customer Testimonial Submission and the Photo Usage Permission Request have no checkbox at all, and their answers are radios. That is a real, dated, searchable record of who agreed to what, and it is enough for most internal consent files — but it is not an executed document and you should not present it as one. If the agreement has to be signed in the legal sense, keep the form as your intake and pass each submission to a signing service: formformform's DocuSign automation fires on new submission and sends an envelope from your template to the person who just filled the form in.
Does a form like this make my consent GDPR-compliant?
It helps with part of it and cannot do the rest on its own. GDPR expects consent to be freely given, specific, informed and as easy to withdraw as it was to give. A form is good at the middle two: name each purpose as its own option rather than one blanket tick, write the purpose and any risks into the form so the answer is informed, and keep the response as evidence of what was shown at the time. Withdrawal is the part the form does not provide. The Photo & Video Consent Form has an option reading "I understand I may revoke this consent in writing at any time", but that is a statement of your policy, not a mechanism — you still need a monitored address for revocations and a way to find and delete the response when one arrives. formformform lets you export or delete individual responses at any time, and the data is hosted in the EU, but the process around it is yours to run.
Can a parent, guardian or owner consent on someone else's behalf?
Yes, on three of these templates, and they go about it differently. The Field Trip Permission Form only ever expects a parent or guardian and asks for their name, email, phone and emergency contact alongside the student's. The Veterinary Treatment Consent Form takes the owner's details and makes them confirm they are the legal owner or authorised agent for the animal. The Photo & Video Consent Form is the one that handles both cases in a single form, with a radio that asks whether the subject is 18 or older or whether a parent is answering, and a guardian-name field underneath. That guardian field is optional as generated and stays visible either way — if you want it required when a parent is answering and hidden otherwise, add the branch in the editor after cloning.
Can I attach a scanned form, an ID, or a countersigned page to the consent record?
Yes, but you have to add the field yourself. formformform's editor has a File Upload section — 2 MB per file, and encrypted along with everything else on an encrypted form — but the template generator cannot seed one, so no template on this page arrives with an upload field on it. Clone the one you want, drag in a File Upload field for the scan or the guardian's ID, and it will work like any other field. If what you want is a link to a shoot gallery or a study protocol rather than the file itself, a Short Text field asking for the URL is lighter and keeps the response small.
