The patient never fills one in. Every template carries two subjects: the animal — species, breed, sex, age, neuter status — and the person answerable for it. Breed is asked almost everywhere, because it sets handling and grooming time before anyone touches the animal.
The rest is safety information: medications and doses, allergies, vaccination status, bite and reactivity history, a microchip number. It exists to be read before the animal is handled, not filed afterwards.
What goes in a veterinary or pet care form?
An animal record, an owner record, and a reason for contact. The animal side runs species — dogs, cats, birds, rabbits, reptiles, equines, wildlife — plus breed, sex, neuter status, weight and microchip number. The owner side runs contact details, an emergency contact, the current vet's number, and whatever needs authorising: a procedure, a vaccination rule, a landlord's approval.

Why use veterinary form templates
Allergies, doses and a bite or reactivity history exist only if the owner writes them down. A flagged dog gets a different handler and a quieter playgroup; a missed medication becomes a clinical problem.
A separate emergency contact, the previous vet's number and a microchip number are what a kennel needs at the moment the owner's phone rings out.
A consent submission holds the named procedure, the spend limit above which you must call, and separate acknowledgements of risk, cost and ownership. A sensitive form can carry end-to-end encryption and sit on EU infrastructure; confirm what your practice requires.

Record both parties: pet name, breed, sex and neuter status against owner contact details and an emergency contact who is not the owner.

Branch with conditional logic: landlord contact only for renters applying to adopt, feeding schedules only for overnight stays, coat and matting detail only for a full groom.

Getting better responses
A medical history box returns 'she's on something for her hips'. A dedicated medications field asking for dosage, with allergies kept separate, gives staff something they can act on.
An owner will write 'nervous around men' in a notes field and never think to mention a bite. A direct yes or no, with a follow-up description, decides who handles the animal.
Microchip numbers and vaccination dates get looked up at home, not recalled at a counter with a lead in one hand. Put the link in the appointment confirmation.

What does a new patient intake form ask a pet owner?
Species, breed, date of birth or approximate age, sex and neuter status, weight, microchip number, vaccination status, current medications with doses, known allergies, and the previous practice's name and number. Finish with a free-text reason for visit, which gives the vet context no dropdown can. Keep vaccination dates optional so a client without their records to hand can still submit.
Is an online treatment consent form binding?
It produces a timestamped submission with the owner's typed name, the procedure described in their file, an emergency spend limit and separate ticked acknowledgements of risk, financial responsibility and legal ownership. That is a far clearer record than a verbal yes at the desk. Whether it satisfies your regulator, insurer or local law is yours to confirm.
How should a shelter run lost and found reports through one form?
Put a lost or found choice at the top, then ask both sides the same description questions — species, colour and markings, last-known location given as cross streets rather than a city, and microchip status. Matching only works when the two halves are directly comparable. Submissions can then be filtered and searched to pair a report from Tuesday with one from Thursday.
Is pet and owner data on these forms secure?
Submissions are encrypted at rest and in transit, end-to-end encryption is available for sensitive forms, EU hosting is available and formformform is GDPR compliant. That matters most for adoption applications and consent forms, which carry home addresses, landlord contacts and spending authorisations. Early access costs nothing and caps neither forms nor responses.
