HVAC Service Quote Request Template

The difference between a diagnostic visit and a repair visit is usually four pieces of information: what the system is, how old it is, what error code it's showing, and when it was last serviced. This form asks for all four before anyone gets in a van, alongside the access questions that decide whether one engineer or two are needed to reach the unit.

About this template

An HVAC service enquiry fails in a specific and expensive way: the engineer arrives, diagnoses the fault, discovers the part is not on the van, and books a second visit. That second visit is the customer's frustration and the company's lost half-day, and it is usually caused by four missing pieces of information — what the system is, how old it is, what code it is showing, and what has already been done to it. This form asks for all four before anyone sets off. The make-and-model field carries an explanation of where to look, because most people have simply never had a reason to read the label on the indoor unit, and the error-code field asks for the code copied exactly, letters included, since manufacturer codes tend to name the failed component directly.

Service history gets three fields rather than one. Whether it has ever been serviced, an approximate date of the last one, and a free-text record of previous repairs and replaced parts. That last field is where a compressor replaced twice in three years shows up, and repeat repairs on the same component are the strongest argument there is for replacing a system rather than fixing it again. A paperwork checklist sits alongside, mostly to surface an in-date manufacturer warranty before a customer pays someone to do work the manufacturer would have covered.

The access block asks about the indoor unit and the outdoor plant separately, because they are rarely equally reachable — a wall unit at floor level can pair with a condenser on a flat roof or in a communal area that needs permission. Whether the property is occupied or trading decides whether the work happens out of hours and at what rate. There is no budget field: the fault, the system age and the access set the price, and asking what someone hoped to spend collects a number that changes none of them. formformform hosts every form in the EU and handles submissions under GDPR; optional end-to-end encryption can be enabled per form if you'd rather site addresses and access details were sealed to your key before storage. The product has no payment field, so nothing here takes a call-out fee or a deposit — that goes wherever you already invoice.

Record uneven room temperatures and weak airflow as symptoms, pointing at duct or balance faults rather than the unit itself.

System Replacement Quote for an Ageing Unit

Use age, repeat-repair history and running-cost complaints to build a replace-rather-than-repair case in writing.

Second Opinion on an HVAC Quote

Take an enquiry from someone who already has a price, with the history and paperwork fields doing the comparison work.

Server Room Cooling Fault Report

Handle a critical space where a cooling failure is an outage risk, flagged by property type and the usable-right-now field.

Adding Cooling to Extra Rooms Enquiry

Scope an extension of an existing multi-split, where the current system type and capacity decide what's possible.

Pre-Purchase HVAC Condition Report Request

Take an inspection-only enquiry from a buyer who needs the system's age and service history assessed before completion.

What's included

System type spanning split, multi-split, ducted, heat pump and boiler — not a single "service type" dropdown
A make-and-model field with an explanation of where to find the label, so parts can be brought on the first visit
A dedicated error-code field, because manufacturer codes usually name the failed component outright
Service history asked as a question, a date and a free-text repair record — the strongest evidence for repair versus replace
Separate access questions for the indoor unit and the outdoor plant, which are rarely equally reachable
A "usable right now" field paired with urgency, so an out-of-action system in a trading kitchen is visible
A paperwork checklist that surfaces an in-date manufacturer warranty before anyone is paid to fix it
No budget field — the fault, the age and the access set the price

How to create an HVAC service quote request

  1. 1

Click "Use this template" to load the HVAC service quote request into your free formformform account.

  2. 2

Trim the system-type options to what you're qualified and certified to work on, rather than declining enquiries after they arrive.

  3. 3

Decide whether you take emergency call-outs. If you don't, remove the emergency option from the urgency dropdown so nobody submits expecting one.

  4. 4

State your call-out fee and any out-of-hours rate in the intro paragraph, since the form can't take payment and the expectation is better set before submission than after.

  5. 5

Add conditional logic in the editor so fault-specific fields — error code, symptoms, usable-right-now — appear only for fault visits, and installation questions only for new work. The template ships flat, with every field visible.

  6. 6

Route submissions by urgency once logic is in place, so an out-of-action system reaches a phone rather than a shared inbox.

  7. 7

Publish and link it from your service page, and give the same link to the landlords and agents who send you repeat work.

Best practices for your HVAC service quote request

Chase the make and model if it arrives blank. One photo of the data plate saves a return visit, and most customers will happily go and take it if you explain why.

Treat the error code as a triage field, not a formality. Codes are manufacturer-specific and usually name the component, which is what turns a diagnostic visit into a repair visit.

Read service history before you quote a repair. A system that's never been serviced has drifted in several ways at once, and quoting only for the reported symptom sets you up to be blamed for the next one.

Flag repeat repairs honestly. The same part failing twice is the moment to talk about replacement, and saying so in writing at quote stage is fairer than a third invoice.

Check the warranty answer before booking chargeable work. An in-date manufacturer warranty changes who pays, and finding out afterwards is a refund conversation.

Ask about both access points separately, and keep them separate. A reachable indoor unit tells you nothing about a condenser on a flat roof or behind a locked communal gate.

Quote out-of-hours work as out-of-hours work. Shops, kitchens and offices that can only shut down at night should see that priced as a line rather than absorbed into a total.

Say whether you're sending an estimate or a fixed price. For a fault, a figure from a form is an estimate

the fixed price follows diagnosis on site, and naming that avoids the argument later.

Frequently asked questions

Why does the form ask for a make and model?

Because it's the single field that most often prevents a second visit. With a model number an engineer can check parts availability and bring what's needed; without it, the first visit is a diagnosis and the repair is booked for another day. The field description says where the label usually is, since most people have never needed to look.

What if the customer doesn't know the system type or the age?

Both fields have a "not sure" option, and the photo link field covers the rest — a picture of the data plate gives model, serial and often refrigerant type in one shot. It's better to let someone submit honestly than to force a guess that sends the wrong engineer.

Why is there no budget field?

Because the fault, the system age and the access set the price, and all three are already on the form. What a customer hoped to spend doesn't change whether a compressor has failed. If you triage by budget, add a dropdown in the editor — but read the fault description and service history first.

Can customers upload a photo of the unit?

The template collects a share link, because formformform's template generator has no upload field to seed. The product does support uploads — add an Upload field to your copy in the editor and customers can attach photos of the data plate or the error display directly.

Can I take a call-out fee or a deposit through this form?

No. formformform has no payment field, and the only payment processing in the product is subscription billing for your own account. State your call-out fee in the intro paragraph so it's clear before someone submits, and collect it however you normally do.

How should emergency requests be handled?

Two fields flag them: "Is the System Usable at All Right Now?" and the urgency dropdown. Once you add logic in the editor you can branch on either, routing an out-of-action system to a phone rather than a shared inbox. Until then, watch the notification email — the fields are there so the answer is visible at a glance.

Does the template ship with conditional logic?

No. Every field is visible from the start, because formformform's template generator writes forms flat. After cloning you can show error code and symptoms only for fault visits, and installation sizing questions only for new work — it's a short change in the editor.

Is this template free?

Yes. Use it, customise it and collect unlimited submissions at no cost, with no per-submission fees.
