Recovery forms handle a purchase that went wrong, anchored on an order number and purchase date. What is a retail form?
Retail forms carry a request between a shop and one of three people: a customer, a supplier prospect or a job applicant. Customer-facing ones anchor on a transaction — the order number, product name and purchase date staff need to find the sale — then ask what went wrong and what should happen next. Trade-facing ones qualify instead, on business type and order volume.

Why use retail form templates
Otherwise they come by email, social message or a call to the shop floor, each missing something different. A required order number means the sale is found before anyone replies.
A request that only says the item was wrong defaults to a refund. Offering exchange, store credit, and on an exchange both sizes plus an alternative colour, keeps it.
Ratings split across cleanliness, staff, selection and checkout speed, tagged with location and date, show which branch has the queue problem. Fixed return reasons do it for stock: one style behind the too-small returns is a size chart to fix.
How to create your own

Start with the fields that identify the purchase — order number, product name, date of purchase — and require the order number.

Ask why from a fixed list of return reasons, then what the customer wants back: refund, exchange or store credit.

Branch with conditional logic: both sizes only on an exchange, packaging condition only where the item may be resold, a resale certificate number only for wholesale enquiries.

Ask for a link to photographs of the fault in a URL field, then publish where the request starts — the returns page, a QR code on the packing slip, a tablet by the exit.
Getting better responses
A wholesale enquiry that never mentions a minimum order value collects window shoppers; a returns form that never states the return window collects requests you have to decline.
Make name and email optional. The remark about the queue at the till is the one worth having, and the one withheld when a form insists on a name.
An application that skips November-to-January availability produces interviews with people who cannot work the trading peak; a registry request with no event date leaves a consultant unable to prioritise.

What does a product return form need to capture?
The order number, product name and date of purchase so staff can find the sale, a reason chosen from a fixed list, and the customer's preferred resolution — refund, exchange or store credit. Add a packaging condition question, since it decides whether the item can be resold as new, and an upload field for photographs of the fault — 2 MB per file is ample for a phone photo. Return windows and refund rights differ by market, so confirm what applies to yours and state it on the form.
How is a size exchange form different from a return form?
A return ends the sale; an exchange keeps it, so the form has to be precise about what is coming back and what goes out. Ask for the current size and the requested size as separate required fields, because "I need a bigger one" always costs a follow-up message. Offering an alternative colour and a store credit fallback covers the common case where the size wanted sells out before the exchange is processed.
How do I collect in-store feedback once the customer has left?
Put the form where the visit ends — a QR code on the receipt, the bag or the exit door — so it is answered within minutes rather than recalled a week later. Keep it to a handful of rated dimensions plus one open comment, and ask which branch and which date the visit was. That is what lets you compare one location against another instead of reading each response on its own.
Can I take a payment through a retail form?
Not yet. Native payment fields are on the roadmap; today the form collects the request and the money moves through the tool you already use — a restocking fee deducted from the refund, a registry deposit taken at the appointment, a wholesale sample invoiced with the first order. State the amount and the terms on the form itself, so the figure is agreed before anyone replies. While formformform is in early access nothing is charged for, nothing is held back and nothing is capped.
