About this template
A social media client onboarding form is the document that turns a signed retainer into a publishable calendar. It is not a campaign brief and not a pitch questionnaire — the work is already won, the channels are already agreed in principle, and what remains is everything that makes daily publishing possible without a phone call: which accounts you are actually being handed, how you get into them, and what you are allowed to say once you are there.
Three of these questions do most of the work. The access question asks to be invited through Meta Business Manager, LinkedIn page admin or TikTok Business Center as a named user, because a partner invitation can be withdrawn cleanly and a shared password cannot. The off-limits question surfaces the topics a client assumes everybody knows to avoid — the litigation, the restructure, the pricing — before a scheduled post finds one of them. And the approval turnaround question sits deliberately next to the cadence question, because a daily commitment against a three-day approval loop is a plan that fails in week two.
The baseline figures are the quietest and most valuable part of the form. Once you start posting there is no way back to an honest starting number, and every growth claim in the first quarterly report rests on a figure somebody wrote down in week one. Capturing followers and engagement rate at onboarding, from the client rather than from your own later screenshot, makes the first report an argument you can win.

Best practices for your social media client onboarding form
Ask for a Business Manager or partner invitation, never a login. Named access can be revoked in a click when the retainer ends; a shared password means a password reset and an awkward email.
Never add a password or 2FA field to this form, even when the client offers. If something genuinely cannot be granted by invitation, move it to a password-manager share or a one-time secret link agreed outside the form.
Record the baseline follower and engagement figures before the first post, not at the first report. A number captured after work has started is not a baseline, and everybody in the room knows it.

Read the cadence and approval answers together. If they are incompatible, fix it in onboarding while it is a conversation, rather than in month two when it is a failure.
Insist on a phone number for the crisis contact. An email address is not a crisis contact, and the first hour is the whole game.
Ask explicitly whether you may reply to complaints. Ambiguity here produces either an agency that ignores an angry customer for a day or one that answers on the brand’s behalf without authority.
Revisit the form at each renewal. Approvers move on, off-limits topics change, and an onboarding answer from eighteen months ago is a liability rather than a record.
Frequently asked questions
How is this different from a social media campaign request form?
A campaign request is per-campaign and internal: it briefs one push with a goal, platforms, post count, hashtags and a paid budget. This form runs once, at the start of a retainer, and covers the standing conditions every campaign after it inherits — account access, voice, banned topics, approval chain, cadence, baselines and a crisis contact. You would send this once and the campaign request many times.
Why does the form never ask for account passwords?
Because it does not need them. Every major platform grants agency access by invitation — Meta Business Manager, LinkedIn page admin, TikTok Business Center — and invited access can be revoked cleanly without anyone rotating a credential. A password in a form is a password in a database, a notification email and somebody's inbox archive. Asking who administers the account, and inviting a named address instead, is both safer and less work.
Can I attach the brand guidelines to this form?
Add an Upload field in the editor after cloning and the client can attach files up to 2 MB each. For a full guidelines PDF a shared link is usually more practical, so the template asks the voice questions directly rather than assuming a document exists — many clients discover while filling this in that theirs does not.
Should the baseline follower count be required?
Yes, and it ships required for that reason. It takes fifteen seconds to look up and it is the only number your first report can honestly be measured against. Engagement rate ships optional, because a guessed engagement rate is worse than a blank one you measure yourself in week one.
What if the client wants us to have full control including replying to complaints?
Then the reply-permissions question records it, and the crisis section becomes more important rather than less. Note who may pause the calendar out of hours and whether a holding statement can be posted without approval — those two answers are what let you act at speed with authority you can point to afterwards.
Is what the client submits kept private?
Submissions travel over HTTPS into your formformform account rather than a shared inbox, end-to-end encryption can be enabled per form so only you can read them, EU hosting is available, and formformform is GDPR compliant. SOC 2 and ISO 27001 are the hosting provider's certifications, not formformform's. None of that is a reason to collect credentials here — the safest handling of a password is not to ask for one.
